股神经阻滞对全麻膝关节置换术患者术后睡眠状态的影响 被引量:7

摘要 目的:探讨股神经阻滞对全麻膝关节置换术患者睡眠状态的影响。方法 :120例择期全膝关节置换术患者随机分为股神经阻滞观察组和对照组,各60例。所有患者采用喉罩全麻,术中使用全凭静脉麻醉维持,股神经阻滞组手术结束后气管拔管前10min超声引导下行患侧股神经阻滞(0.3%罗哌卡因30ml),两组患者术毕均连接静脉自控镇痛泵。分别收集患者气管拔管后30min,术后1d,术后2d在静止及运动关节时的视觉模拟疼痛评分(VAS),并收集术前,术后1d,术后2d阿森斯失眠量表失眠评分。结果:两组术后气管导管拔除后30min,两组VAS评分无统计学差异(P>0.05)。手术后两组的VAS评分均有所下降。但观察组的VAS评分在术后1d及术后2d均低于对照组(P<0.05)。两组术前失眠评分没有统计学意义(P>0.05)。较之术前,术后两组患者失眠评分均有所升高,观察组失眠评分升高无统计学意义(P>0.05),对照组失眠评分升高有统计学意义(P<0.05)。术后1d和术后2d两组各自失眠评分无明显变化,但观察组评分明显低于对照组(P<0.05)。结论:股神经阻滞镇痛能够改善全麻术后镇痛,减少术中术后阿片类药物的需求,改善睡眠功能紊乱。
