探讨手术联合嗓音训练对伴肌紧张性发声障碍声带息肉患者的疗效

Study on Clinical Curative Effect of Operation and Voice Training on the Patients with Muscle Tonue Pararthria Vocal Cord Polyp

目的探讨手术联合嗓音训练对伴肌紧张性发声障碍声带息肉患者的临床疗效。方法方便选取2015年1月—2016年1月来该院治疗伴肌紧张性发声障碍声带息肉患者98例,并将其随机分为手术组与联合组。手术组行单纯手术治疗,联合组行手术联合嗓音训练治疗。另选取同时间段来该院进行健康体检且无嗓音疾病人员49名,为对照组。利用Xion声学分析软件对对照组、手术组以及联合组患者治疗前后的嗓音基频微扰(Jitter)、嗓音障碍指数(DSI)、振幅微扰(Shimmer)进行测试,并对其进行比较。结果相比较治疗前,治疗后手术组、联合组的Jitter、Shimmer值均得到显著降低,其中手术组的Jitter从治疗之前的1.35下降为0.83;联合组相关指标从1.38下降为0.58;Shimmer手术组从之前的6.76降低为术后的4.65;联合组相关指标从术前的6.8降低为2.9,DSI显著升高(P<0.05)。而联合组的Jitter和Shimmer值降低更多,且DSI值升高更多。治疗后,手术组Jitter和Shimmer值均高于对照组,DSI值均低于对照组(P<0.05)。治疗后,联合组的Jitter和Shimmer值与对照组相比较,差异无统计学意义(P>0.05)。结论对伴肌紧张性发生障碍声带息肉患者行喉显微外科手术联合嗓音训练,可使得患者术后嗓音质量得到有效提高,临床价值较高,值得在临床上推广应用。

Objective To study the clinical curative effect of operation and voice training on the patients with muscle tonue pararthria vocal cord polyp. Methods 98 cases of patients with muscle tonue pararthria vocal cord polyp treated in our hospital from January 2015 to January 2016 were selected and randomly divided into two groups, the operation group and the combined group were respectively treated with operation and operation combined with voice training, and 49 cases of healthy physical examination persons in our hospital at the same period were selected as the control group, and the Jitter,DSI, Shimmer of the three group were tested before and after treatment and compared. Results The Jitter and Shimmer values in the operation group and combined group were obviously decreased, and the Jitter in the operation group decreased from 1.35 to 0.83, and the related index in the combined group decreased from 1.38 to 0.58, and the Shimmer decreased from 6.67 to 4.65 in the operation group, and the related index in the combined group decreased from 6.8 to 2.9 before operation, and the DSI obviously increased（P〈0.05）, and the decrease of Jitter and Shimmer values in the combined group was more, and the DSI value increased more, after treatment, the jitter and shimmer values were obviously higher than those in the control group, and the DSI value was lower than that in the control group（P〈0.05）, and the DSI value was lower than that in the control group, after treatment, the differences in the Jitter and Shimmer values between the operation group and the control group were not statistically significant（P〉0.05）. Conclusion The operation and voice training on the patients with muscle tonue pararthria vocal cord polyp can effectively improve the voice quality of patients with higher clinical value, which is worth clinical promotion and application.
