慢性乙型肝炎患者免疫球蛋白检验的临床探讨

Clinical Study on Immunoglobulin Test of Patients with Chronic Hepatitis B

目的探讨慢性乙型肝炎患者检验免疫球蛋白(Ig)的临床意义。方法方便选取2014年12月—2016年12月该院门诊及住院收治的56例慢性乙型肝炎患者为实验组,另择取同期56名健康体检者为对照组,两组患者均于清晨空腹抽取静脉血测定IgG、IgA、IgM水平并进行比较。依据病症将实验组患者分为轻度乙肝组、中度乙肝组和重度乙肝组,对比观察3组IgG、IgA、IgM水平差异;经针对性治疗后,根据预后将实验组患者分为好转组、稳定组和进展组,对比观察3组IgG、IgA、IgM水平差异。结果验组IgG(13.4±1.7)g/L,IgA(3.5±0.6)g/L,IgM(2.7±0.4)g/L,各项指标均高于对照组(P<0.05)。实验组乙肝病情越严重,IgG、IgA、IgM指标值越高。治疗后,IgG、IgA、IgM水平随患者病症改善情况而变化,好转者明显下降,进展者进一步上升。结论疫球蛋白是能够反映慢性乙型肝炎病症严重程度及预后的敏感指标,对临床诊治慢性乙型肝炎具有重要参考价值。

Objective To discuss the clinical significance of immunoglobulin test of patients with chronic hepatitis B.Methods Convenient selection 56 cases of patients with chronic hepatitis B admitted and treated in our hospital from December 2014 to December 2016 were selected as the experimental group, 56 cases of healthy physical examination persons at the same period were selected as the control group, and the IgG, IgA, IgM levels at fasting in the morning of the two groups were measured, and the patients in the experimental group were divided into the mild group, moderate group and severe group, and the differences in the IgG, IgA, IgM levels between the three groups were compared and after the targeted treatment, the patients in the experimental group were divided into the improved group, steady group and progress group,and the differences in the IgG, IgA, IgM levels of the three groups were observed and compared. Results The IgG（13.4±1.7）g/L,IgA（3.5±0.6）g/L, IgM（2.7±0.4）g/L in the experimental group were higher than those in the control group（P〈0.05）,and the severer the hepatitis B, the higher the IgG, IgA, IgM values, after treatment, the IgG, IgA, IgM levels changed with the improvement of diseases of patients, and the improved patients obviously decreased and the progress patients further increased. Conclusion Immunoglobulin is the sensitive index of reflecting the severity of chronic hepatitis B and prognosis,which is of important reference value in the clinical diagnosis and treatment of h chronic hepatitis B.
