生化检验指标在病毒性肝病诊断中的价值与特点

Value and Features of Biochemical Test Indexes in Diagnosis of Viral Liver Diseases

目的探讨生化检验指标在病毒性肝病诊断中的价值与特点。方法方便选取2015年12月—2016年8月该院收治的75例病毒性肝病患者为实验组,另择取同期75名健康志愿者为对照组,全部患者均于清晨空腹采血进行血生化检测,对比观察生化指标TBIL、AST、CHE、ALT、ADA、LAP、PA。结果实验组TBIL(166.3±21.5)μmmol/L,AST(257.3±40.1)U/L,CHE(2.2±0.3)k U/L,ALT(362.6±31.3)U/L,与对照组比较差异有统计学意义(P<0.05)。慢性肝炎ADA(41.2±6.3)U/L、肝炎后肝硬化ADA(44.7±5.9)U/L、肝癌LAP(482.6±35.4)mg/d L、肝硬化PA(108.4±11.7)mg/L、肝癌PA(103.0±12.6)mg/L,与其它病毒性肝病指标比较差异有统计学意义(P<0.05)。结论 TBIL、AST、CHE、ALT是临床诊断病毒性肝病的可靠指标,而ADA、LAP和PA则有助于临床鉴别肝病类型及判断预后。

Objective To discuss the value and features of biochemical test indexes in diagnosis of viral liver diseases.Methods Convenient selection 75 cases of patients with viral liver diseases admitted and treated in our hospital from December 2015 to August 2016 were selected as the experimental group, and 75 cases of healthy volunteers at the same period were selected as the control group, and all patients underwent the blood biochemical test by fasting blood collection in the morning, and the biochemical indexes such as TBIL, AST, CHE, ALT, ADA, LAP, PA were compared and observed.Results The TBIL（166.3±21.5）μmmol/L, AST（257.3±40.1）U/L,CHE（2.2±0.3）k U/L, ALT（362.6±31.3）U/L in the experimental group were different from those in those in the control group（P〈0.05）, and the chronic hepatitis ADA（41.2±6.3）U/L,post-hepatitis cirrhosis of liver ADA（44.7 ±5.9）U/L, liver cancer LAP（482.6 ±35.4）mg/d L, hepatic cirrhosis PA（108.4 ±11.7）mg/L, liver cancer PA（103.0±12.6）mg/L were different from the other viral liver disease indexes（P〈0.05）. Conclusion TBIL,AST,CHE,ALT are the reliable indexes in clinical diagnosis of viral liver diseases, and the ADA,LAP and PA contribute to the clinical differential diagnosis of liver disease type and determination of prognosis.
