吸入布地奈德辅助治疗儿童肺炎支原体肺炎的疗效分析

Clinical effect of inhaled budesonide auxiliary treatment for children with mycoplasma pneumoniae pneumonia

目的探讨吸入糖皮质激素(ICS)辅助治疗儿童肺炎支原体肺炎的临床疗效及安全性。方法选取肺炎支原体肺炎患儿218例,随机分为常规组和吸入激素组,每组109例。常规组给予红霉素静滴,合并细菌感染者加用头孢菌素抗感染,重症加用甲强龙静滴,同时辅助口服止咳化痰药物,护理上加强拍背利于痰液排出。治疗组在常规治疗基础上加用布地奈德,每次1mg,每天2次,雾化吸入治疗,疗程7天。比较两组患儿临床症状改善情况、临床疗效、治疗前后血清CRP水平及治疗期间不良反应发生情况。结果治疗组咳嗽明显减轻时间、退热时间、肺部啰音消失时间、住院时间均明显短于常规组、总有效率明显优于常规组,经比较差异有统计学意义(P均<0.01)。两组患儿治疗期间均未见明显不良反应。结论吸入糖皮质激素辅助治疗儿童肺炎支原体肺炎可有效缓解临床症状,促进肺部炎症吸收。

Objective To explore the clinical effectiveness and safety of inhaled ICS auxiliary treatment for children with mycoplasma pneumoniae pneumonia. Methods 218 children with mycoplasma pneumonia were randomly divided into the normal group and the hormone group,109 cases in each group. The normal group was given erythromycin,the patients complicated with bacteria infection were given cephalosporin anti-infection treatment,and severe cases with methylprednisolone. Auxiliary oral cough expectorant drugs were given at the same time,and backslapping nurse was strengthened to discharge sputum. The hormone group was additionally treated with budesonide,every time 1 mg,twice every day for 7 days. Before and after treatment,their serum CRP levels,clinical symptoms,clinical effect and adverse reactions were compared. Results The time of cough relieving,pyretolysis,lung rale disappearing,and hospital stay was obviously shorter in the hormone group than in the normal group,and the total effective rate was significantly better in the hormone group than in the normal group( P < 0. 01). There was no obvious adverse reactions occurred during treatment in the two groups. Conclusion Inhaled corticosteroids auxiliary treatment for children with mycoplasma pneumoniae pneumonia has good curative effect,which can effectively relieve clinical symptoms and promote lung inflammation absorption.
