正常胰腺宽体探测器单源双能CT灌注成像初探 被引量:3

The Primary Study of Wide Detector Single-Source Dual Energy CT Perfusion Scan in Normal Pancreas

摘要 目的探讨宽体探测器单源双能CT胰腺灌注成像的可行性及正常胰腺组织的灌注特点。方法搜集27例行上腹部CT增强及胰腺CT灌注扫描且胰腺无病变者,按性别和年龄进行分组,采用全器官一次轴位扫描模式。测量胰腺头部、体部、尾部的血流量(BF)、血容量(BV)、平均通过时间(MTT)、达峰时间(TP)和表面通透性(PS)。由两名放射科医师独立完成上述测量。记录并计算辐射剂量。结果两名观察者测得所有灌注参数一致性良好(ICC均>0.75)。正常胰腺头部、体部和尾部的不同参数之间差异均无统计学意义(P>0.05)。不同年龄组(≥60岁、<60岁)及不同性别组胰腺组织血流灌注参数之间差异均无统计学意义(P>0.05)。灌注期有效辐射剂量为14.47 mSv。结论宽体探测器单源双能CT可以用单次轴扫完成单器官容积成像,实现胰腺的低辐射剂量连续灌注成像。正常胰腺组织的血流灌注参数与性别、年龄及胰腺的不同部位无关。 Objective To explore the feasibility of low-dose whole-pancreas imaging utilizing wide detector singlesource dual energy CT,and to study the perfusion characteristics of the normal pancreas. Methods Twenty-seven patients( male 12,female 15,with a mean age of 56. 6 ± 13. 6 years,ranging from 22 to 82) without pancreatic disorders who received pancreatic CT perfusion imaging were analyzed. They were grouped according to age and sex. The whole-pancreas CT perfusion was performed on the Revolution CT scanner with axial scan protocol. Blood flow( BF),blood volume( BV),mean transit time( MTT),time to peak( TP) and permeability of surface( PS) on the head,body and tail of pancreas were measured. Two radiologists independently completed the measurement. Effective radiation dose was calculated. Results The consistency between the two investigators in all perfusion parameters was good( all ICC〉0. 75). There was no significant statistical difference for BF,BV,MTT,TP and PS of the head,body and tail of the normal pancreas. There was no statistical difference in all perfusion parameters between different age groups( ≥60 years old, 60 years old)( P〉0. 05). No Statistically significant differences in all perfusion parameters were noted between male and female groups( P〉0. 05). The effective radiation dose of the CT perfusion protocol was 14. 47 mSv. Conclusion Wide detector singlesource dual energy CT with maximum detector width of 16 cm can achieve whole-organ volume imaging by single axial scanning,proving that low-dose whole-pancreas consecutive perfusion imaging is feasible. All the perfusion parameters of the normal pancreas have no obvious relation with gender,age and different pancreatic areas.
