乡镇卫生院开展医养结合服务的内在制约因素与对策——以山东烟台市乡镇卫生院为例

The Internal Restrictive Factors and Countermeasures of Combination of Medical and Health Care in Township Hospitals——A case study of township health centers in Yantai

目的:通过对乡镇卫生院管理人员对开展医养结合服务意愿及问题进行问卷调查,探究影响乡镇卫生院开展医养结合服务的内在制约因素,为提升乡镇卫生院医养结合服务质量提供参考。方法:采用多阶段分层抽样的方法,抽取30个乡镇卫生院202个管理人员为调查对象,利用自行设计的调查问卷进行现场调查。结果:乡镇卫生院开展医养结合服务过于注重外因而忽略内因的关键作用;对开展医养结合服务发展思路与定位不够清晰;开展医养结合服务基础设施建设滞后;开展医养服务专业人才缺乏;服务模式单一,大部分乡镇卫生院主要以开展义诊的形式来开展医养结合服务。结论:乡镇卫生院发展医养结合服务应该着力于内在制约因素的解决,在认知理念、规划布局、专业设施建设和人才队伍建设、发展模式进行进一步变革,通过医养结合服务水平的提升助推乡镇卫生院实施转型发展。

Objectives：Through carrying out questionnaires to the managers of township hospitals to survey their attitudes and difficulties towards the combination of medical and health care to explore the internal factots confined the launch of this service and to provide reference for the improvement of the quality of the combination of medical and health care in township hospitals. Methods ： By using multi - stage stratified sampling method, 202 managers from 30 township hospitals were selected as the subjects of the study, and the questionnaire was designed by ourselves. Result：The township hospitals over highlighted the external factors during the launch of combination of medical and health care ;while overlooked the internal ones which also played a crucial role; Besides, their develop idea and position are not clear enough and the infrastructure is relative left behind;The lack of professionals and the single mode for service remines unsolved and most of the township hospitals carry out the combination of medical and health care mainly through non - profitable treat. Conclusion ： the development of the service of combination of medical and health care should focus the solution of the integral confined factors . and intensify the reform in cognitive idea, planning and layout, the construction of professional equipment and talent group, as well as the development mode. Accelerate the transformation of town- ship hospitals by the improvement of the quality of combination of medical and health care.
