患者在入院时,对患者信息进行详细询问非常重要,特别对服药情况更要如实掌握。同时对于患者正在使用的自备药物要纳入医院统一管理,避免患者出现重复服药,确保患者安全。指出医患有效沟通是落实患者安全的前提条件,鼓励患者参与患者安全工作,自备药规范管理是确保用药安全工作的重要一环。

It is essential to perform a detailed consultation and evaluation for the patient＇s information and situation, especially for his/her drug administration since hospital admission. Additionally, an optimized management of the prescription which also taking the patient personal-administered drugs into consideration would effectively avoid drug abuse and available to patient safety. It is considered that effective doctor patient communication is the precondition of patient safety. It also shows that patient participation and personal administrated drug standardized management is a key point for patient safety management.
