我院门诊处方不合理用药调查及分析

Investigation and Analysis of Irrational Use of Outpatient Prescriptions in Our Hospital

目的调查门诊处方不合理用药情况,促进合理用药。方法调查2016年9月至12月医院门诊处方20 910张,统计其中不合理用药的情况,并分析不合理用药处方,统计不合理用药处方科室来源及其类型分布。结果医院门诊20 910张处方中,不合理用药处方1 194张(5.71%),科室来源:9 347张外科处方中不合理460张(4.92%);10 542张内科处方中不合理704张(6.68%);1 021张其他科处方中不合理30张(2.94%);内科不合理用药处方发生率最高,外科其次,其他科室处方不合理发生率较低;不合理用药处方类型:用药方式不合理201张,使用药物不合理166张,药物配伍及联合用药不合理412张,药物滥用174张,用药剂量不合理130张,用药时间不合理111张。结论医院门诊处方存在明显不合理用药状况,且门诊处方不合理用药类型较多,临床需重视门诊处方不合理用药状况,及时实施针对性干预措施,提升药物治疗安全性。

Objective To investigate the irrational use of drugs in outpatient prescriptions so as to promote rational drug use.Methods From November to December 2016,20 910 outpatient prescriptions were investigated,and the irrational use of drugs was ana lyzed. The irrational prescriptions were analyzed,the departments and types of irrational prescriptions were statistically analyzed.Results Among the 20 910 outpatient prescriptions in the hospital,there were 1 194 prescriptions（ 5. 71%） for irrational use of drugs. The prescriptions were mainly involved in these departments： in the 9 347 surgical prescriptions,460 prescriptions（ 4. 92%） were unreasonable. In the 10 542 medicine prescriptions,704 prescriptions（ 6. 68%） were unreasonable. In the 1 021 other prescriptions,30 prescriptions（ 2. 94%） were unreasonable. The incidence rate of irrational prescriptions in the department of medicine was the highest,followed by the department of surgery,which were lower in the other departments. The types of irrational prescription： 201 prescriptions for the irrational way of using drugs,166 prescriptions for irrational use of drugs,412 prescriptions for drug compatibility and irrational combination of drugs,174 prescriptions for drug abuse,130 prescriptions for irrational dosage,111 prescriptions for irrational time of using drugs. Conclusion The outpatient prescriptions in our hospital have obvious irrational drug use situation and many types of irrational drug use,the irrational drug use in outpatient prescriptions should be paid attention to in clinical,and the targeted interventions should be implemented in a timely manner to improve the safety of drug treatment.
