宫颈癌超声造影的诊断价值及与Egr-1基因表达的相关性 被引量:6

摘要 目的探讨宫颈癌患者的超声造影特征及其与Egr-1基因表达的相关性。方法分析40例病理确诊为宫颈癌患者的经腹超声造影特征,观察肿瘤的形态结构、血流信号分布及多普勒血流特征,对术后的肿瘤组织应用QRTPCR检测Egr-1基因的表达,分析超声造影表现与Egr-1基因表达之间的相关性。结果肿瘤组织的峰值强度(PI)为(75.71±2.13)显著高于正常组织(58.12±1.92),差异具有统计学意义(P<0.001)。肿瘤组织的病灶达峰时间(TTP)为(32.43±1.76)s显著短于正常组织(40.41±2.24)s,差异具有统计学意义(P=0.003);Egr-1基因在宫颈癌组织标本中的表达明显低于癌旁组织,差异具有统计学意义(P<0.001);Egr-1基因的表达与宫颈有无肿物,肿块的大小(≤4 cm)、淋巴结转移及病理分级相关(P均<0.05);宫颈癌造影增强强度与Egr-1表达相关,与肿瘤大小、淋巴结转移及病理分级无明显相关关系(P>0.05);非均匀性增强在Egr-1低表达患者中的比率明显高于Egr-1高表达者,差异具有统计学意义(P<0.05)。结论宫颈癌超声造影特征可显示其与Egr-1基因表达的相关性,有助于无创性评估宫颈癌的预后评估。 Objective To investigate the correlation between contrast-enhanced ultrasound (CEUS) and Egr-1 gene expression in patients with cervical carcinoma.Methods CEUS of 40 patients with cervical cancer was analyzed.The tumor morphological feature,distribution,and characteristics of Doppler blood flow were correlated with the expression of Egr-1.Results The peak intensity (PI) of tumor (75.71±2.13) was significantly higher (P〈0.001) than that of normal tissue (58.12±1.92) whereas the time to peak (TFP,32.43± 1.76) was significantly shorter (P=0.003) than that of normal tissues (40.41±2.24).The Egr-1 gene expression of cervical cancer was significantly lower than that of adjacent tissues (P〈0.001) and was significantly correlated with tumor size 〈4 cm,lymph node metastasis,and higher pathological grade (P〈0.05).The degree of contrast enhancement of cervical cancer was correlated with the expression of Egr-1 and did not correlate significantly with tumor size,lymph node metastasis,and pathological grade (P〉0.05).Heterogeneous enhancement was significantly more frequent at low than high Egr-1 expression (P〈0.05).Conclusion CEUS is useful to evaluate the prognosis of cervical cancer.
