广州地区分枝杆菌菌株库资源分析 被引量:5

摘要 目的提高广州地区分枝杆菌菌株库建设水平,并大力促进其应用水平。方法应用分枝杆菌菌株库信息系统资料,对2009至2016年保存菌株的病例来源和流动性特征、分枝杆菌菌种类型、结核分枝杆菌耐药性特点进行分析。结果7年间,该库共保藏了来自12110例患者的15854株分枝杆菌菌株。12110例患者来自我国31个省、直辖市及特别行政区的567个地级市辖区/县和其他4个国家,其中常住人口6522例、流动人口2411例、资料不详3177例,分别占患者总数的53.86%、19.91%和26.23%,每例患者保藏1。13株菌株不等。在15854株分枝杆菌菌株中,对11068株分枝杆菌进行了菌种鉴定,鉴定出结核、牛、堪萨斯、猿、海、亚洲、戈登、瘰疠、苏尔加、鸟、胞内、玛尔摩、蟾、土、次要、龟、脓肿、偶然、耻垢、微黄、草分枝杆菌等21个分枝杆菌菌种;对8853株结核分枝杆菌进行了异烟肼、利福平、乙胺丁醇、链霉素、阿米卡星、左氧氟沙星、莫西沙星等7种抗结核药物的体外药物敏感性试验测定,获得95种不同的耐药类型,其中包括5157株全部药物敏感株、1798株非耐多药株、1674株耐多药株和224株广泛耐药株。结论广州地区分枝杆菌菌株库保藏菌株病例来源地域辽阔并具有相当的流动性,分枝杆菌菌种与结核分枝杆菌耐药类型资源丰富,具有较高的临床和科研应用价值。但有约1,4的保藏菌株的患者信息资料不完整,其患者信息资料库建设水平有待进一步提高。 Objective To improve the level of Mycobacteria biobank construction in Guanzhou and promote its application vigorously. Methods Based on the data from Guangdong Mycobacteria Culture Collection Information System (GDMCC) of Mycobacteria biobank, the district information and floating characteristics of patients, from which various Mycobacteria were isolated from 2009 to 2016, and the species of Mycobacteria and the drug resistance to anti-tuberculosis drugs of Mycobacteria Tuberculosis were analyzed. Results A total of 15 854 strains of various Mycobacteria from 12 110 patients, which came from four foreign countries and 567 municipal districts or counties of 31 provinces in China, were frozen. These patients were categorized as census register, floating population, and unknown according to the differences between the census register and the place of residence. The number of cases of census register, floating population, and unknown were 6 522, 2 411, and 3 177, which accounted for 53.86%, 19.91%, and 26.23%, respectively. And the number in the range of one to thirteen of strains isolated from clinical samples of different time were frozen. Of all the strains, 11 068 strains were identified and covered 21 species of Mycobacteria, as following: M.tuberculosis, M.bovis, M.kansasii, M.simiae, M.marium, M.asiaticum, M.gordonae, M.scrofulaccum, M.szulgai, M.avium, M.intracellulare, M.malmoense, M.xenopi, M.terrae, M.triviale, M.chelonae, M.abscessus, M.fortuitum,M.smegmatis, M.flavescens, and M.phlei. Drug susceptibility to seven anti-tuberculosis drugs (including soniazid, rifampicin, ethambutol, streptomycin, amikacin, levofloxacin and moxifloxacin) of 8 853 M.tuberculosis strains were tested. Ninety-five different types of drug susceptibility were found. They included 5 157 sensitive strains to all drugs, 1 798 non-multi-drug resistance-tuberculosis strains ( NMDR-TB ) , 1 674 multi-drug resistance tuberculosis strains (MDR-TB), and 224 extensive drug resistance tuberculosis trains(XDR-TB). Conclusion There is a wealth of resources and information available about clinical and scientific research in this Mycobacteria biobank. But, it is essential that the collection of case information must be enhanced further based on the fact that the current information of about quarter of all cases is incomplete.
