基于根源性分析法提高手术间一次性无菌物品管理合格率

Increase Disposable Sterile Operating Room Management Qualification Rate Based Root-cause Analysis

目的分析手术间内一次性无菌物品管理质量低的原因,列出改善措施实施质量改进,提高一次性无菌物品管理合格率。方法运用根源性分析法(RCA)结合鱼骨图,找出一次性无菌物品不合格的近端原因并进行根因查证,针对根本原因列出改善计划逐步实施。结果护士主动参与,切合临床实际的改进措施易于执行,改善后手术间一次性无菌物品管理质量较改善前明显提高。结论根源性分析法简单实用,护理人员易于掌握和应用,着眼于系统,可找出组织或流程设计中的缺陷,有利于护理质量持续改进。

Objective Analyzing the causes of low disposable sterile operating room management quality improvement measures listed in the implementation of quality improvement, increase disposable sterile goods management yield. Methods Root-cause analysis （RCA） in conjunction fishbone diagram to identify reasons for the proximal end of the disposable sterile and failed to verify the root cause, the fundamental reason for the gradual implementation of improvement programs are listed. Results Nurses active participation, in line with clinical practice improvements easy to perform, after improving significantly improved operating room before disposable sterile than the quality of management improvement. Conclusion Simple root-cause analysis and practical, easy＇to grasp and nursing applications, focusing on the system, identify the organization or process design flaws in the quality of care in favor of continuous improvement.
