温针灸治疗原发性痛经即刻镇痛效应临床观察

Immediate Analgesic Effect of Warming Needle Moxibustion for Primary Dysmenorrhea

目的:评价月经期温针灸治疗寒凝血瘀型原发性痛经患者的即刻镇痛效应。方法:将44例寒凝血瘀型原发性痛经患者,随机分为治疗组和对照组,每组22例。治疗组于月经第1天针刺气海、关元、三阴交,留针同时行温针灸,治疗3个月经周期。对照组于月经第1天口服布洛芬缓释胶囊1粒,治疗3个月经周期。采用视觉模拟评分量表(visual analogue scale,VAS)观察治疗前、治疗后各时间段患者的疼痛强度。连续治疗3个月经周期后观察临床疗效。结果:温针灸治疗痛经从施治15 min开始起效,口服布洛芬缓释胶囊从30 min开始起效,温针灸施治后30 min镇痛效果最明显,口服布洛芬缓释胶囊60 min镇痛效果最明显,两组治疗后60 min、90 min、120 min、150 min、180 min镇痛效果比较,差异无统计学意义(P>0.05);治疗3个月经周期,治疗组疼痛强度明显降低,且显著优于对照组(P<0.05),治疗组有效率86.36%,明显优于对照组的9.09%,差异有统计学意义(P<0.05)。结论:温针灸治疗寒凝血瘀型原发性痛经起效快。长期疗效优于口服布洛芬缓释胶囊,能代替非甾体抗炎药物治疗原发性痛经。

Objective：To evaluate the immediate analgesic effect of warming needle moxibustion for primary dysmenorrhea with Cold Blood Stasis Syndrome.Methods：44 patients with primary dysmenorrheal of Cold Blood Stasis Syndrome were randomly divided into treatment group and control group,with 22 cases in each group.From the first day of the menstrual cycle,the treatment group was treated with acupuncture Qihai,Guanyuan and Sanyinjiao acupoints and then the needles were retained warmly,the control group was treated with one Ibuprofen sustained release capsule per day.The treatment lasted for three menstrual cycles.The visual analogue scale（VAS）was used to observe patient＇s pain intensity in each time before and after treatment.After continuous treatment of three menstrual cycles the curative effect was observed.Results：Warm needle acupuncture in the treatment group worked at the 15thminute and the most obvious analgesic effect took place at the 30thminute,while that of the control at the 30thminute and the most obvious analgesic effect took place at the 60th minute.There were no statistical difference between the warm acupuncture group and the Ibuprofen group at 60 min,90 min,120 min,150 min,and 180 min after treatment（P〈0.05）;After three menstrual cycles of treatment,the analgesic effect of the acupuncture group was significantly better than that of the control group（P〈0.05）.The total effective rate（86.36%）of the treatment group was significantly better than that（9.09%）of the control group（P〈0.05）.Conclusion：Warming needle acupuncture treatment for primary dysmenorrhea with cold Blood Stasis Syndrome can function quickly and the long-term curative effect is significantly better than that of the oral ibuprofen sustained release capsule group.Warming needle acupuncture treatment can take the place of non-steroidal anti-inflammatory drug treatment in patients with primary dysmenorrhea.
