摘要
目的:探讨血清瘦素(Leptin)、白细胞介素-18(IL-18)与稳定期慢性阻塞性肺疾病(COPD)患者营养不良发生的相关性及其机制。方法:测定66例稳定期COPD患者及20例正常人的营养情况参数,以放射免疫法测定血清Leptin水平,ELISA法测定与IL-18水平;分析血清Leptin水平与营养参数及血清IL-18水平相关性。结果:稳定期COPD患者血清Leptin水平与营养参数呈显著正相关,且营养不良组血清Leptin水平显著低于非营养不良组与正常对照组;营养不良组血清IL-18水平高于非营养不良组,均显著高于对照组,而血清Leptin水平与IL-18水平无显著相关性。结论:血清Leptin与稳定期COPD患者营养参数相关,且COPD营养不良组血清IL-18水平升高,因此Leptin-细胞因子联合作用可能是营养不良发生的原因之一。
Objective：To explore the significance of serum leptin and IL-18 in malnutrition of stable COPD patients. Method：Serum leptin and IL-18 levels were measured by radioimmunoassay and ELISA in 66 patients with stable COPD and 20 healthy controls. The correlation between leptin and nutritional parameters was analyzed. Result：Serum leptin levels were positively correlated with nutritional parameters in 66 patients（P〈0.05）. Serum leptin concentrations were significantly correlated with nutritional parameters in both malnourished patients with COPD and non-malnourished patients（P〈0.05）.The leptin level was significantly lower in malnourished group than in non-malnourished group（P〈0.05）. Serum IL-18 level was significantly higher in malnutrition group than in nonmalnutrition group and healthy group（P〈0.05）.Serum leptin level was not significantly correlated with IL-18 level in patients with COPD.Conclusion：Serum leptin and IL-18 are associated with nutritional parameters in patients with COPD.Therefore, the combined effect of Leptin-cytokines may be one of the causes of malnutrition in patients with stable COPD..
