摘要
目的:探讨血清瘦素(Leptin)、白细胞介素-18(IL-18)与稳定期慢性阻塞性肺疾病(COPD)患者营养不良发生的相关性及其机制。方法:测定66例稳定期COPD患者及20例正常人的营养情况参数,以放射免疫法测定血清Leptin水平,ELISA法测定与IL-18水平;分析血清Leptin水平与营养参数及血清IL-18水平相关性。结果:稳定期COPD患者血清Leptin水平与营养参数呈显著正相关,且营养不良组血清Leptin水平显著低于非营养不良组与正常对照组;营养不良组血清IL-18水平高于非营养不良组,均显著高于对照组,而血清Leptin水平与IL-18水平无显著相关性。结论:血清Leptin与稳定期COPD患者营养参数相关,且COPD营养不良组血清IL-18水平升高,因此Leptin-细胞因子联合作用可能是营养不良发生的原因之一。
Objective:To explore the significance of serum leptin and IL-18 in malnutrition of stable COPD patients. Method:Serum leptin and IL-18 levels were measured by radioimmunoassay and ELISA in 66 patients with stable COPD and 20 healthy controls. The correlation between leptin and nutritional parameters was analyzed. Result:Serum leptin levels were positively correlated with nutritional parameters in 66 patients(P〈0.05). Serum leptin concentrations were significantly correlated with nutritional parameters in both malnourished patients with COPD and non-malnourished patients(P〈0.05).The leptin level was significantly lower in malnourished group than in non-malnourished group(P〈0.05). Serum IL-18 level was significantly higher in malnutrition group than in nonmalnutrition group and healthy group(P〈0.05).Serum leptin level was not significantly correlated with IL-18 level in patients with COPD.Conclusion:Serum leptin and IL-18 are associated with nutritional parameters in patients with COPD.Therefore, the combined effect of Leptin-cytokines may be one of the causes of malnutrition in patients with stable COPD..
