股前外侧皮瓣急诊修复伴动脉缺损的手部创面

Emergency repair of hand wound with artery defect using anterolateral thigh flap

目的探讨股前外侧Flow-through皮瓣急诊修复伴动脉缺损手部创面的临床效果。方法自2013年5月至2015年8月，我们共收治了13例手部创面伴动脉缺损的患者，采用股前外侧Flow-through皮瓣血管蒂桥接动脉重建血液循环的同时修复创面。皮瓣切取面积为8cm×7cm～21cm×12cm。结果术后13例皮瓣及供区植皮全部存活，随访时间为6～15个月，平均10个月，皮瓣质地、色泽良好，恢复部分感觉，患者对手功能满意。按中华医学会手外科学会上肢部分功能评定试用标准评定：优7例，良4例，可2例。结论对于伴肢（指）体动脉缺损的手部创面，股前外侧Flow—through皮瓣修复创面的同时可利用血管蒂桥接缺损动脉，是一种较理想的修复方法。

Objective To investigate the clinical application of flow-through anterolateral thigh perforator flap for repair of hand wound with artery defects. Methods From May 2013 to August 2015, 13 cases of hand wound with artery defects were treated by flow-through anterolateral thigh perforator flap. The flap areas ranged from 8 cm×7 cm to 21 cm×12 cm. Results All 13 flaps and donor sites survived uneventfully. All the patients were follow-up for 6 to 15 months with an average of 10 months. The texture and color of flaps were good. Partial protective sensation of flaps recovered. All the patients were satisfied with hand function. According to upper extremity function evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association, the results were graded as excellent in 7 cases, good in 4 cases and fair in 2 cases. Conclusion It may be an ideal method to use flow-through anterolateral thigh perforator flap for the treatment of hand wound with artery defects. It can bridge the vessel defects and repair the soft tissue defects in one stage.
