腹腔镜和开腹手术治疗老年子宫内膜癌疗效的Meta分析

MATA ANALYSIS FOR TREATING ENDOMETRIAL CARCINOMA IN ELDERLY PATIENTS WITH LAPAROSCOPIC AND OPEN SURGERY IN CHINA

目的:系统评价腹腔镜与开腹手术治疗我国老年子宫内膜癌的疗效。方法:计算机检索万方全文数据库、中国知网全文数据库、维普中文期刊数据库、中国生物医学文摘数据库,根据纳入标准筛选文献。从文献中提取数据,采用RevMan 5.2软件进行统计学分析。结果:纳入5篇临床对照研究(CCT)。与开腹手术(LPT)相比,腹腔镜手术(LPS)组手术时间短,术中失血量少,术后排气时间早,术后留置尿管时间短,住院时间短,差异有统计学意义(P<0.05);而清除淋巴结数量、手术并发症发生率比较差异无统计学意义(P>0.05)。结论:腹腔镜手术创伤小、恢复快,且不增加手术并发症,是治疗我国老年子宫内膜癌的理想术式。

Objective： To systematically evaluate the curative effects for minting endometrial carcinoma in elderly patients with laparoscopic and open surgery in China Methods： The literatures were collected according to selection standard in WanFang database,CNKI, VIP and CBM; and the clinical trials （CCT） were statistically analyzed by RevMan 5.2. Results： 5 cases of CCT were analyzed in this study. Compared with open surgery, the lapamscopic surgery had shorter operation time, less intraoperative blood loss, early postoperative exhaust time, shorter postoperative indwelling catheter time and shorter hospital stays （P〈 0.05）; But there was no statistical significanee in number of lymph nodes clearance and surgical complications （P〉 0.05）. Conclusions： Lapamscopic surgery is an ideal operation in the treatment of endometrial carcinoma in elderly patients in China, because it has the advantages of less-injury, quick recovery and without increasing complications.
