摘要 目的探讨适用于基层医疗机构干预过度输液用药的模式。方法选择4家不同等级医院开展监测-培训-计划(MTP)模式干预门诊过度输液,调查干预前后门诊处方静脉输液率等指标,并进行统计分析。结果经过4轮MTP干预,4家医院门诊输液都呈下降趋势,降幅为25%~43%;另外,抗菌药物使用率也有所下降,降幅在12%~21%,均具有统计学差异(P<0.05)。结论通过MTP模式实施输液使用指征等策略,能降低各基层医疗机构门诊输液使用率,表明MTP干预模式具有普遍适用性,适合不同等级的医院减少门诊过度输液。 Objective To investigate the mode of intervention for excessive transfusion in the basic medical institutions. Methods Four different levels of hospitals were selected to carry out the MTP model to intervene outpatient clinic excessive infusion,and the indexes of intravenous transfusion rate were analyzed before and after the intervention. Results After 4 rounds of MTP intervention,the infusion rate was decreased in the outpatient department of the four hospitals,and the decreasing amplitude was between 25%-43%. In addition,the use of antimicrobial drugs also decreased,the decreasing amplitude was between 12%-21%,all of the data had statistical difference( P〈0. 05). Conclusion The outpatient clinic infusion rate in the basic medical institutions was reduced through MTP mode implementation of transfusion indications and other strategies, it shows that MTP intervention model has universal applicability and is suitable for different grades of hospitals to reduce outpatient excessive infusion.
