多普勒超声心动图评价左室负荷状态的右室舒张功能

RIGHT VENTRICULAR DIASTOLIC FILLING IN RIGHT VENTRICULAR OVERLOAD: A PULSED DOPPLER ECHOCARDIOGRAPHIC STUDY

本文应用三尖瓣多普勒频谱方法,研究各种右室负荷状态的先心病右室舒张功能的变化。观察到右室压力负荷过重患者舒张早期与晚期右室充盈指标均有显著改变,右室容量负荷过重者以舒张晚期功能异常为主,而右室尚未出现负荷过重的先心病也存在右室舒张功能异常现象。

One hundred and sixty-three patients with or without right ventricular overload were studiedby pulsed Doppler echocardiographic tricuspid flow. The tricuspid spectrum in diastole was used to assess the filling condition of right ventricle. Significant changes both in early and late diastole were observed, in eases with right ventricular pressure overload.Doppler derived parameters of late filling period elevated in the group of right ventricular volume overload. Some indexes of right ventricular inflow in patients without right ventricular overload also changed. Good correlation was found between Doppler tricuspid data (Ai, A, Ei/Ai, E/A) and right ventricular mean and/or diastilic pressure.It is concluded that there are significant changes in both early end late diastolic filling in cases with right ventricular pressure overload and abnormal late diastolic function in patients with right ventricular volume overload. Cases of congenital heart diseases without ventricular overload also demostrate impairment of right ventricular filling.
