摘要
目的 通过对周围型肺癌宝石能谱CT增强扫描,探讨其多参数分析与不同病理分型的相关性.方法 收集病理证实的周围型肺癌43例,其中鳞癌24例,腺癌19例,均采用宝石能谱CT行GSI扫描,通过GSI viewer软件取得病灶能谱曲线、碘基图,并计算标准化碘浓度及40~70 kev能谱曲线斜率,并与不同病理分型进行回顾分析.结果 感兴趣区(ROI)碘基图中碘含量鳞癌、腺癌分别为(24.4±4.5)、(16.5±5.4)μg/cm3;40~70 kev斜率分别是4.7±0.2、3.3±0.3;标准化碘含量分别为0.23±0.03、0.19±0.01;差异均有统计学意义(P〈 0.05).结论 周围型肺癌中鳞癌和腺癌其能谱CT碘含量、能谱曲线斜率及标准化碘含量不同,可以术前利用能谱CT对肺鳞癌与腺癌进行预判.
Objective To investigate the correlation of the multi-parameter analysis with different pathological types through the enhancement CT scan of peripheral lung cancer.Methods Forty three patients with pathologically confirmed peripheral lung cancer were selected, including 24 cases of squamous cell carcinoma and 19 cases of adenocarcinoma.Patients underwent GSI scan by gemstone spectral imaging.The focus energy spectrum and iodine figure were obtained.The standardized iodine concentration and 40-70 kev energy spectrum curve slope were calculated.A retrospective analysis was conducted for the different pathological types.Results The iodine contents in the region of interest (ROI) were (24.4±4.5)μg/cm3 and (16.5±5.4)μg/cm3 in squamous cell carcinoma and adenocarcinoma.The 40-70 kev slopes were 4.7±0.2 and 3.3±0.3.The standardized iodine contents were 0.23±0.19 and 0.03±0.01.The differences were significant (P〈 0.05).Conclusions The CT iodine contents, energy spectrum curve slopes and standardized iodine contents are different between the peripheral lung cancer and adenocarcinoma.The preoperative energy spectrum CT can be used to distinguish the lung squamous cell carcinoma and adenocarcinoma.
