原发性骨质疏松症中的骨密度与骨代谢生化指标相关性分析 被引量:19

摘要 目的探讨骨代谢生化指标和骨密度在原发性骨质疏松症中的相关性。方法选择2015年1月至2016年12月期间在广东医科大学附属医院住院的151例原发性骨质疏松症患者为骨质疏松组。选择同期无骨质疏松患者101例为无骨质疏松组。测量骨密度,并检测两组患者血清中的25-羟基维生素D(25-OH-VD)、N-端骨钙素(N-MID)、β-胶原特殊序列(β-CTX)、总I型胶原氨基酸延长肽(t-PINP)水平,测定生化指标值并统计分析。结果骨质疏松组骨密度和25-OH-VD水平明显低于无骨松组,差异有统计学意义(P<0.05),而骨质疏松组患者血清中N-MID、β-CTX、t-PINP水平与无骨质疏松组比较,差异均无统计学意义(P>0.05)。骨质疏松症组男性骨密度和血清25-OH-VD水平明显高于女性,差异均有统计学意义(P<0.05);骨质疏松症组女性与男性患者比较NMID、β-CTX、t-PINP的水平差异均无统计学意义(P均>0.05)。无骨质疏松组中患者骨密度和血清25-OH-VD、NMID、β-CTX、t-PINP水平男女之间的差异均无统计学意义(P>0.05)。骨质疏松症组中25-OH-VD与骨密度和血清β-CTX、t-PINP有明显相关性(P均<0.05),与N-MID无明显相关性(P>0.05)。结论骨代谢生化指标灵敏性强且具有无创伤性,结合骨密度测定,能够全面、合理评价骨转换,有利于原发性骨质疏松症的治疗。 Objective: To explore the the relationship between bone metabolism biochemical markers and bone mineral density (BMD) in primary osteoporosis . Methods: A total of 151 cases of primary osteoporosis (OP)andl01 eases without osteoporosis (N-OP)were recruited from January 2015 to December 2016, hospital patients in guandung medical university affiliated hospital. Bone mineral density was measured and serum25-OH-VD, N-MID, β-CTX, t-PINP levels were deteeted. The biochemical index values were analyzed statistically. Results : The bone mineral density and level of the serum 25-OH- VD in OP group were significantly lower than those of N-OPgroup (P 〈 0. 05 ). But the N-MID, β-CTX, t-PINP levels in ser- um between two groups had no statistic difference (P 〉 0. 05 ). In the OP group, bone mineral density(BMD) and serum 25- OH-VDin male were significantly higher than those infemale ( P 〈 0. 05 ). The N-MID, β-CTX, t-PINP between different gender had no statistic difference( P 〉 0. 05 ). In N-OPgroup, bone mineral density and levels of the serum 25-OH-VD, N- MID, β-CTX, t-PINP were not statistically different ( P 〉 0. 05 ). In OP group, theserum 25-OH-VDwas obvious related with- bone mineral density (BMD) and 13-CTX, t-PINP( P 〈 0. 05 ), but not related with N-MID ( P 〉 0. 05 ) Conclusion: Bone me- tabolism Biochemical indicators have a sensitive, non-invasive advantages, combined with the determination of bone mineral density can be a comprehensive and reasonable evaluation of bone turnover, and is conducive to the treatment of primary osteoporosis.
