中药、针刺治疗中风后丘脑痛的Meta分析

A Meta-analysis on Traditional Chinese Medicine and Acupuncture for Post-Stroke Thalamic Pain

目的:系统评价中药、针刺治疗中风后丘脑痛的临床疗效。方法:通过计算机和人工方法检索关于中药、针刺治疗中风后丘脑痛的临床研究文献,收集符合要求的随机对照试验(RCT)文献,运用Cochrane系统评价的方法,使用Rev Men 5.3软件进行Meta分析。结果:共纳入10篇文献,总计627例中风后丘脑痛病例,结果显示中药、针刺治疗中风后便秘的总有效率优于对照组[总有效率:OR=3.32,95%CI(2.25~4.90),Z=6.02,P<0.000 01;针刺治疗有效率:OR=2.72,95%CI(1.29~5.73),Z=2.62,P=0.009;恶化率:OR=0.24,95%CI(0.08~0.69),Z=2.63,P=0.009],差异有统计学意义。结论:中药、针刺治疗中风后丘脑痛有效,对照其他治疗方法有一定的优势,但纳入文献中RCT文献质量较低,还需高质量、大样本、多中心的随机临床研究用以进一步验证。

Objective：The clinical efficacy of traditional Chinese medicine and acupuncture for the post-stroke thalamic pain was systematically reviewed.Methods：By computerized and manual retrieval of clinical research literature regarding acupuncture and traditional Chinese medicine for the post-stroke thalamic pain,the randomized control trials（RCTs） that met the inclusive criteria were collected.Cochrane systematic review method was used and Revmen 5.3 software was adopted to perform this Meta analysis.Results：Totally 10 articles were included,involving 627 cases of the thalamic pain.As a result,the total effective rate of traditional Chinese medicine and acupuncture for the thalamic pain constipation was significantly superior to those of the control group [total effective rate：OR=3.32,95% CI（2.25-4.90）,Z=6.02,P〈0.000 01;effective rate for acupuncture：OR=2.72,95% CI（1.29-5.73）,Z=2.62,P=0.009;degradation rate：OR=0.24,95% CI（0.08-0.69）,Z=2.63,P=0.009].Conclusion：Traditional Chinese medicine and acupuncture are effective for the post-stroke thalamic pain and have some advantages compared with other therapies.But the quality of included RCTs was low and high-quality,large-sample and multi-center RCTs are needed to perform further verification.
