血小板、纤维蛋白原、前列腺特异抗原联合检测在前列腺癌诊断中的意义 被引量:3

Clinical value of the combined detection of platelet count,fibrinogen and PSA in the diagnosis of prostate cancer

摘要 目的分析血小板计数(PLT)、纤维蛋白原(FIB)以及PSA相关检测(t-PSA、f-PSA、f/t-PSA)在诊断前列腺癌中的意义。方法采集研究对象的外周静脉血,分别检测PLT、FIB、t-PSA、f-PSA,并对结果进行统计分析。结果前列腺癌组的PLT、FIB、t-PSA、f-PSA显著高于对照组和良性前列腺增生组,差异有统计学意义(P<0.05);而前列腺癌组与良性前列腺增生组f/t-PSA差异无统计学意义(P>0.05);良性前列腺增生组的f/t-PSA显著低于对照组,差异有统计学意义(P<0.05),良性前列腺增生组的PLT、FIB、t-PSA、f-PSA与对照组差异无统计学意义(P>0.05)。PLT、FIB、f/t-PSA的ROC曲线下面积(AUC)分别为0.648、0.656、0.744,约登指数分别为0.287、0.361、0.528;f/t-PSA分别联合PLT、FIB以及3种联合的AUC分别为0.807、0.782、0.828,约登指数分别为0.547、0.539、0.585。结论单独检测PLT、FIB、f/t-PSA对前列腺癌具有一定的诊断意义,3种联合检测可以提高诊断效能。 Objective To analyze the clinical value of platelet count( PLT),fibrinogen( FIB),and PSA relevant values( t-PSA,f-PSA,f/t-PSA) in the diagnosis of prostate cancer. Methods Peripheral blood specimens of objects were collected from the study objects for the detection of PLT,FIB,t-PSA,f-PSA,and the detection results were conducted for statistical analysis. Results Levels of platelet count,fibrinogen,t-PSA and f-PSA were higher than those in the control group and BPH group,with the differences statistically significant( P〈0. 05),but the differences on f/t-PSA levels had no statistical significance between the prostate cancer and BPH( P〈0. 05). The f/t-PSA level in BPH was higher than that in the control group,with the differences statistically significant( P〈0. 05),and the difference on the platelet count,fibrinogen,t-PSA and f-PSA levels displayed no statistical significance between BPH group and the control group( P〈0. 05). Areas under the ROC curve( AUC) of PLT,FIB and f/t-PSA were 0. 648,0. 656,0. 744,respectively. Youden index were 0. 287,0. 361,0. 528,respectively. AUC of f/t-PSA combined with PLT,FIB and combination of the three values were 0. 807,0. 782,0. 828,respectively. Youden index were 0. 547,0. 539,0. 585,respectively. Conclusion Single detection of PLT,FIB and PSA relevant values is meaningful for the diagnosis of prostate cancer to some extent,while the combined detection of f/t-PSA,PLT and FIB can improve the diagnostic efficiency.
