摘要
目的:观察降脂抗纤方治疗非酒精性脂肪肝(NAFLD)的临床疗效。方法:将80例NAFLD患者随机分为对照组30例和治疗组50例。对照组采用多烯磷脂酰胆碱治疗,治疗组采用复方降脂抗纤方治疗。治疗12周,观察2组治疗前后症状与体征、肝功能、血脂指标、血清肿瘤坏死因子-α(TNF-α)、血清转化生长因子-β1(TGF-β1)和瘦素(Leptin)水平变化。结果:总有效率治疗组为86.0%,对照组为70.0%,2组比较,差异有统计学意义(P<0.05);2组主要症状体征改善、血清ALT、GGT、TG、HDL-C、LDL-C、TNF-α、TGF-β1和Leptin指标变化治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05)。结论:降脂抗纤方治疗非酒精性脂肪肝疗效显著,其作用机制可能与其抑制炎性因子TNF-α、TGF-β1和Leptin的释放有关。
Objective: To observe the clinical efficacy of antihyperlipidemic and antifibrotic prescription in the treatment of non-alcoholic fatty liver disease( NAFLD). Methods: Eighty NAFLD patients were randomly divided into control group( n = 30) and treatment group( n = 50). The control group was treated with polyene phosphatidylcholine,while the treatment group was treated with compound antihyperlipidemic and antifibrotic prescription. The changes in symptoms and signs,liver function,blood lipid parameters,and serum levels of tumor necrosis factor-α( TNF-α),transforming growth factor-β1( TGF-β1),and leptin after 12 weeks of treatment were evaluated. Results: The treatment group had a significantly higher overall response rate than the control group( 86. 0% vs70. 0%,P〈0. 05). After treatment,both groups showed significant improvements in main symptoms and signs,serum levels of alanine aminotransferase,gamma-glutamyl transpeptidase,triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,TNF-α,TGF-β1,and leptin( all P〈0. 05),and there were significant differences in the above indices between the two groups( P〈0. 05). Conclusion: Antihyperlipidemic and antifibrotic prescription has significant efficacy in the treatment of NAFLD,possibly by inhibiting the release of TNF-α,TGF-β1,and leptin.
