4800张门、急诊中成药处方点评与分析及干预对策

Review and Analysis on 4800 Prescriptions of Chinese Patent Medicine from Outpatient and Emergency Department

目的:了解洛阳市第一中医院(以下简称"我院")门、急诊中成药处方的特点并进行点评,了解不合理用药情况。方法:采用回顾性分析法,抽取我院2015年1月—2016年12月年门、急诊中成药处方4 800张,对其中不合理处方进行点评与分析。结果:4 800门、急诊张中成药处方中,不合理处方232张(占4.83%),主要包括不规范处方和用药不适宜处方。其中,不规范处方91张(占39.22%),主要为使用西医诊断和(或)缺中医病名、无证型,慢性病、老年病或特殊情况下需要适当延长处方用药未注明理由;用药不适宜处方141张(占60.78%),主要为适应证不适宜、用法与用量不适宜、重复用药等。结论:我院门、急诊中成药处方存在一些不合理现象,原因较多,临床应加强医、药、患的多方合作,采取针对性的干预措施,促进临床合理用药。

OBJECTIVE： To review the prescriptions of Chinese patent medicine from outpatient and emergency department in Luoyang the Second Hospital of Traditional Chinese Medicine （TCM） （hereinafter referred to as ＂our hospital＂） , and to investigate the irrational application of drugs. METHODS： Retrospective analysis method was adopted, 4 800 prescriptions Chinese patent medicine from outpatient and emergency department from Jan. 2015 to Dec. 2016 were extracted, the irrational prescriptions were reviewed and analyzed. RESULTS： Of the 4 800 prescriptions, there were 232 irrational prescriptions （4.83%）, mainly including non-standard prescriptions and inappropriate prescriptions. Meanwhile, the non-standard prescriptions were 91 （39.22%）, mainly performed as using the diagnosis of western medicine and （or） lack of disease of TCM, without types of TCM syndromes, without reasons for the extension of prescribed medication in chronic disease, senile diseases or under exceptional circumstances. The inappropriate prescriptions were 141 （60. 78% ）, mainly focusing on inappropriate indications, improper usage and dosage, improper repeated medication, etc. CONCLUSIONS： There are some irrational phenomenon in prescriptions of Chinese patent medicine from outpatient and emergency department, which are induced by various causes. The clinic are suppose to strengthen the cooperation among doctor, pharmacist and patient and take intervention measures, so as to promote the rational drug application in clinic.
