摘要
目的探讨原发性高血压患者血清γ-谷氨酰转肽酶(gamma-glutamyltransferase,GGT)水平与代谢综合征(metabolic syndrome,MS)及其组分之间的相关性,为原发性高血压人群代谢综合征的预防控制提供参考依据。方法采用横断面研究,对原发性高血压患者进行体格检查和实验室生化检测,分析GGT与MS组分的关系。结果原发性高血压患者GGT升高检出率为25.02%,患者中超重或肥胖、高血糖、高甘油三酯血症、低高密度脂蛋白血症、代谢综合征者分别占45.04%、23.45%、35.72%、19.04%和33.66%。GGT升高组超重或肥胖、高血糖、高甘油三酯血症、低高密度脂蛋白血症和代谢综合征检出率均高于GGT正常组,差异均有统计学意义(P<0.05)。多因素回归分析结果显示,GGT升高组发生高血糖、高甘油三酯血症、低高密度脂蛋白血症和代谢综合征的风险分别为GGT正常组的1.435倍、1.562倍、1.557倍和1.861倍。随着代谢综合征组分个数的增加,GGT升高检出率呈升高趋势(χ_(趋势)~2=16.978,P=0.000)。结论原发性高血压人群中GGT水平与代谢综合征及其组分密切相关,血清GGT水平升高增加了代谢综合征及其组分的发生风险。GGT可作为代谢综合征发生风险的预测因子。
Objective To investigate the association of serum gamma-glutamyltransferase( GGT) and metabolic syndrome( MS),and provide measures for the prevention and control of MS among patients with primary hypertension. Methods Cross sectional study was used,physical examination and laboratory tests were performed for patients with primary hypertension. Results The detection rate of high GGT was 25. 02%. Among the 1 019 patients,the prevalence of overweight or obesity,hyperglycemia,hypertriglyceridemia,low/high density lipoprotein cholesterol and MS was 45. 04%,23. 45%,35. 72%,19. 04% and 33. 66% respectively. The prevalence of overweight or obesity,hyperglycemia,hypertriglyceridemia,low/high density lipoprotein cholesterol and MS in the high GGT group was higher than those of the GGT-normal group. Compared with the GGT-normal group,the risk of hyperglycemia,hypertriglyceridemia,low/high density lipoprotein cholesterol and MS in the high GGT group increased by 1. 435,1. 562,1. 557 and 1. 861 respectively. With the increasing number of components of MS,the prevalence of high GGT increased significantly( χ~2= 16. 978,P = 0. 000). Conclusion Serum GGT level was closely associated with MS and its components. Serum GGT might be a predictor of MS in patients with primary hypertension.
