大庆市龙凤社区糖代谢异常的横断面研究

A cross sectional study on impaired glucose metabolism in Longfeng community of Daqing

目的了解黑龙江省大庆市龙凤社区人群糖代谢异常水平、相关因素及糖尿病(diabetes mellitus,DM)的知晓率、治疗率和控制率。方法对黑龙江省大庆市龙凤社区6 048例35岁及以上的人群进行横断面调查。结果该人群空腹血糖受损(impaired fasting glucose,IFG)检出率为4.53%(274/6 048),其中男性为4.72%(120/2 543),女性为4.39%(154/3 505);糖尿病患病率为10.88%(658/6 048),其中男性为10.62%(270/2 543),女性为11.07%(388/3 505)。年龄、文化程度、体质量指数(body mass index,BMI)、饮酒、血压和糖尿病家族史是糖代谢异常的危险因素,OR(95%CI)值分别为1.398(1.302~1.502)、1.101(1.017~1.192)、1.314(1.183~1.461)、1.731(1.328~2.257)、2.236(1.914~2.613)和1.247(1.176~1.323),而体力劳动强度和睡眠是糖代谢异常的保护因素,OR(95%CI)值分别为0.914(0.846~0.988)和0.643(0.546~0.757)。糖尿病知晓率为72.80%(479/658),治疗率为80.17%(384/479),控制率为2.92%(14/479)。结论黑龙江大庆市龙风社区糖尿病的患病率及空腹血糖受损检出率处于较高水平,而糖尿病的知晓率、治疗率和控制率较低,提示黑龙江省龙凤社区尚需加大糖尿病知识的宣传力度,提高糖尿病的一、二级预防,提高糖尿病的控制率。

Objective To understand the current status of impaired glucose metabolism and it＇s related factors, as well as the awareness, treatment and control rate of diabetes among residents in Longfeng community of Daqing. Methods A cross over sectional was carried out of the 6 048 residents over 35 years in the l.ongfeng community of Daqing. Results A cross sectional survey was conducted among 6 048 residents age over 35 years old in Longfeng community of Daqing. Chi square test and regression method were used for data analy sis. Results overall, the detection rate of impaired fasting glucose （IFG） was 4.53% （274/6 048） with 4.72% （120,/2 543） in men and 4.39% （154/3 505） in women, the overall prevalence of diabetes mellitus （DM） was 10.88% （658/6 048）, with 10.62% （270/2 543） in men and 11.07% （388/3 505） in women. Multivariable logistic regression analysis showed that age, education, body mass index （BMI） , drinking, blood pressure and family history of diabetes were the risk factors for impaired glucose metabolism with theOR （95%CI）of 1.398 （1.302-1.502）, 1.101 （1.017-1.192）, 1.814 （1.183 1.461）, 1.731 （1.328- 2. 257）, 2. 236 （1. 914-2. 613） and 1. 247 （1. 176 1. 323）, respectively. While physical activity and sleep were the protective .factor for impaired glucose metabolism with the OR （95%CI） of 0. 914 （0. 846-0. 988） and 0. 643 （0. 546-0. 757） .The awareness rate of diabetes was 72.80%（479/658）, the treatment rate was 80. 17% （384/479）, and the control rate was 2. 92% （14/479） . Conclusions In this community, the de- tection rate of IFG and the incidence of DM are relatively higher while the awareness, treatment and control rates of DM are relatively lower, suggesting that it is essential to strengthen the health education about diabetics to increase the control rate of the disease in Longfeng community of Daqing.
