TURP术后并发血栓性疾病影响因素的多元回归分析

Multivariate regression analysis of influencing factors of thrombotic diseases after TURP

目的:探讨经尿道前列腺电切术(TURP)后并发血栓性疾病的影响因素以提高手术安全性。方法:回顾性分析山西医科大学第一医院2011年10月~2016年10月收治行TURP的患者611例,术后新并发血栓性疾病的26例患者作为病例组,采用1∶2配对的病例对照研究方案,从术后未新发生血栓性疾病患者中随机抽取52例作为对照组。进行单因素和多因素条件Logistic回归分析其危险因素。结果:611例患者中术后新并发血栓的发生率为4.25%,单因素分析结果表明,血栓性疾病史、高血压、吸烟史、手术时间、卧床时间、糖尿病和术中低血压可能是术后新并发血栓的危险因素(P<0.05)。多因素条件Logistic回归分析显示,有血栓性疾病史的患者术后新并发血栓性疾病的风险最大(OR=5.146>1)。结论:血栓性疾病史、高血压、糖尿病、吸烟史、卧床时间、术中低血压和手术时间与TURP术后新并发血栓性疾病风险呈正相关,其中有血栓性疾病史的患者术后新并发血栓性疾病的发生率更高。

Objective：To explore the influencing factors of thrombotic diseases after TURP to improve the safety of operation.Method：A retrospective analysis was used to analyze 611 BPH patients admitted to the department of urology of First Medical College of Shanxi Medical University from October 2011 to October 2016.Among them,26 patients with complications of thrombotic disease were selected as the study group.Other 52 patients without complication of thrombotic disease were selected as the control group.Univariate and multivariate conditional logistic regression were used to analyze the risk factors.Result：The results of univariate analysis showed that there was statistically significant difference in thrombotic disease history,hypertension,smoking history,operation time,bedridden time,diabetes and intraoperative hypotension in the study group（P〈0.05）.Multivariate conditional logistic regression analysis showed that the thrombotic disease history was statistically significant（OR=5.146〉1）.Conclusion：The history of thrombotic disease,hypertension,diabetes,smoking history,bedridden time,intraoperative hypotension and operation time were positively correlated with the risk of new thrombotic disease after TURP.Among them,thrombotic disease history led to a higher incidence of postoperative thrombotic disease.
