腹腔镜Ⅲ期结肠癌根治术与开腹手术的临床疗效比较

Clinical efficacy comparison of laparoscopic and open radical resection for the stage Ⅲ colon cancer

目的:比较Ⅲ期结肠癌行腹腔镜手术与开腹手术的临床疗效。方法:根据病例纳入排除标准,筛选2013年1月至2016年6月收治的176例Ⅲ期结肠癌患者,其中112例行腹腔镜手术(腹腔镜组),64例行开腹手术(开腹组)。对比分析两组患者的临床疗效。结果:两组患者年龄、性别、肿瘤部位、病理类型差异均无统计学意义。两组手术时间、淋巴结清扫数量、并发症发生率差异无统计学意义;腹腔镜组手术切口长度、术后肠功能恢复时间、术后住院时间短于开腹组,术中出血量少于开腹组,手术费用、住院总费用高于开腹组。结论:腹腔镜下Ⅲ期结肠癌手术是安全、可行的,具有手术创伤小、术中出血少、术后康复快的微创优势。

Objective：To compare the clinical efficacy of laparoscopic and open radical resection of the stage Ⅲ colon cancer.Methods：According to the case inclusion and exclusion criteria,176 eligible patients of colon cancer were selected from Jan.2013 to Jun.2016,including 112 cases of laparoscopic surgery（LG） and 64 cases of traditional open surgery（OG）.The clinical efficacy of the 2 groups were compared.Results：There were no statistically significant differences at age,gender,tumor location,histological type,operation time,the number of lymph node dissection or incidence rate of complication between LG and OG.LG was less than OG in the index of the length of incision,the time of postoperative intestinal function recovery,postoperative hospitalization time and blood loss.But cost of surgery and hospitalization in LG group was more than that in OG group.Conclusions：The laparoscopic radical resection of the stage Ⅲ colon cancer is a kind of effective and safe technique.It has mini-invasive advantages of fewer operation trauma,less blood loss and faster postoperative recovery.
