内翻型膝关节骨关节炎腓骨近端截骨术与人工全膝关节置换术治疗的短期效果对比分析 被引量:3

Short term effects of proximal fibular osteotomy and total knee arthroplasty in the treatment of varus knee osteoarthritis

摘要 目的:分析内翻型膝关节骨关节炎腓骨近端截骨术与人工全膝关节置换术治疗的短期效果.方法:随机选择2015年1月~2017年1月期间,我院收治的142例内翻型膝关节骨关节炎患者,根据手术方法的不同,将患者具体分为观察组和对照组,其中观察组患者71例,采取腓骨近端截骨术治疗,而对照组患者71例,并采取人工全膝关节置换术,在对2组患者进行3个月的短期治疗后,对2组患者VAS评分与膝关节HSS评分,手术时间和住院时间等指标进行对比分析.结果:对全部患者随访时,观察组和对照组相对较,前者的手术时间和住院时间等指标均低于后者,且组间对比差异存在统计学意义(P<0.05);与术前对比,2组患者术后的VAS评分与膝关节HSS评分均得到明显改善(P<0.05),而组间对比不具有统计学意义(P>0.05).结论:当对内翻型膝关节骨关节炎患者进行临床治疗时,腓骨近端截骨术与人工全膝关节置换术治疗的短期效果相对比较接近,但腓骨近端截骨术更具失血少和创伤小等优点,具有临床推广价值. Objective: to analyze the short - term effect of proximal fibular osteotomy and total knee arthroplasty in the treatment of varus knee osteoarthritis. Method: randomly selected in January 2015 to January 2017, 142 cases of varus knee osteoarthritis patients in our hospital, according to different surgical Methods, the patients will be divided into experimental group and control group, 71 cases of patients in the experimental group, take the proximal fibular osteotomy for the treatment, while the control group of 71 patients, and take total knee arthroplasty, in the short term treatment for 3 months, two groups of patients after the score for the two groups of patients with VAS score and knee joint HSS, operation time and hospitalization time were compared. Result: all patients were followed up, the experi- mental group and the control group relatively, the operation time and hospitalization time and other indicators were lower than the latter, and the difference between groups was statistically significant compared to the presence ( P 〈 0.05 ) ; compared with the preoperative, two groups of patients with postoperative VAS score and HSS knee score were significantly improved ( P 〈 0.05 ), and the comparison be- tween the two groups was not statistically significant ( P 〉 0.05 ). Conclusion: the clinical treatment of varus knee osteoarthritis patients, the short - term effect of proximal fibular osteotomy with artificial total knee arthroplasty is relatively close, but the proximal fibular osteotomy with advantages of less blood loss and less invasive and has clinical value
