奥施康定配合射频消融术治疗骨转移癌痛的效果观察

Effect of oxycodone combined with radiofrequency ablation in treating bone mestastatic cancer pain

目的探讨奥施康定配合射频消融术治疗骨转移癌,对骨转移癌痛患者疼痛缓解、镇痛药用量和生活质量的影响。方法选取我科住院治疗的骨转移癌痛患者90例,每组45例,A组给予口服镇痛药和射频消融术治疗,B组仅给予口服镇痛药治疗。记录患者治疗前、出院时、出院1个月的VAS评分、生活质量评分;记录各时点每天镇痛药用量;记录治疗过程中不良反应的发生情况。结果 45例患者均顺利完成手术,两组患者治疗后疼痛均得到改善(P<0.05),出院时、出院1个月的VAS评分B组明显高于A组(P<0.05);生活质量评分A组明显高于B组(P<0.05);A组奥施康定、吗啡用量少于B组(P<0.05);B组恶心呕吐、嗜睡、瘙痒不良反应高于A组;所有患者无严重并发症发生。结论奥施康定配合射频消融术治疗骨转移癌痛效果明显,提高了癌痛患者的生存质量,减少阿片类药物用量及副作用,推荐临床应用。

Objective To observe the effect of oxycodone combined with radiofrequency ablation in the treat- ment of bone metastatic and evaluate influence of pain relief, drug dosage and quality of life. Methods 90 bone mesta- static patients were randomly divided into two groups ,45 cases in each group ,patients in group A received oxycodone combined with radiofrequency ablation, and patients in group B received oxycodone only. Pain score, quality of life, drug dosage and adverse reactions in pro-treatment, discharge time and 1 month after discharge were recorded. Results Forty-five patients were all successfully operated. After treatment, pain of the two groups were well controlled. Dis- charge time and 1 month after discharge, the VAS score were higher in group B than that of group A （ P 〈 0. 05 ）, the quality of life of the group A were significantly improved than that of group B （ P 〈 0. 05 ）, oxycodone and morphine dosage of group A were lower than that of group B （ P 〈 0. 05 ）. The incidence of nausea and vomiting, drowsiness, pru- ritus were higher in group B than that of group A （P 〈 0.05 ）. There was no serious complication in the two groups. Conclusion Oxycodone combined with radiofrequency ablation used for bone metastatic cancer pain can significantly improve patients＇ quality of life,reduce the dosage of opioids and side effects ,it is recommended to be used in clinic.
