摘要
目的探讨红细胞分布宽度(RDW)与冠心病合并阻塞性睡眠呼吸暂停(OSA)患者严重程度关系。方法将165例OSA患者根据睡眠呼吸暂停低通气指数(AHI)分为对照组(AHI<5次/h)37例,轻度OSA组(5≤AHI≤15次/h)35例,中度OSA组(15<AHI≤30次/h)50例,重度OSA组(AHI>30次/h)43例。采用血液分析仪测定患者RDW,同时采用胶乳增强免疫透射比浊法测定高敏C反应蛋白(hs-CRP)水平。比较不同组患者RDW和hs-CRP水平,分析RDW与OSA患者AHI相关性。结果重度组患者RDW(14.3%)明显高于中度组(13.6%)、轻度组(13.3%)和对照组(12.4%),四组之间比较差异有统计学意义,(P<0.05);对照组、轻度组、中度组和重度组患者hs-CRP水平分别为3.5、3.9、5.0和6.1 mg/L,组间差异有统计学意义(P<0.05);RDW与AHI、氧减指数、hs-CRP呈正相关(r值分别0.375、0.276和0.182,P均<0.05),Logistic回归分析显示RDW是OSA的独立危险因素之一(OR=1.56,95%CI:1.231~2.012,P<0.05)。结论 RDW是OSA的独立危险因素之一,并可作为OSA严重程度评价指标之一。
Objective To investigate the relationship between the red cell distribution width(RDW)and disease severity in coronary artery disease(CAD)patients with obstructive sleep apnea syndrome(OSA). Methods A total of 165 patients with confirmed CAD according to AHI were divided into four groups:control group(AHI〈5/h),mild group(5≤AHI≤15/h),moderate group(15〈AHI≤30/h)and severe group(AHI〉30/h). The RDW and high sensitivity C-reactive protein(hs-CRP) were measured by automated blood cell counter and Latex-enhanced immunity transmission turbidity method , respectively. The RDW values and levels of hs-CRP in different groups were compared ,and the correlation between RDW and AHI was analyzed. Results The RDW values were found to be higher in the severe OSAS group(14.3%)than in the control group(12.4%),mild group(13.3%)and moderate group(13.6%)and this was statistically significant(P〈0.001). When the four groups were compared ,severe group had the higher levels of hs-CRP[control group(3.5 mg/L),mild group (3.9 mg/L),moderate group(5.0 mg/L),and severe group(6.1 mg/L)]. There was a statistically significant correlation between RDW and AHI(r=0.375,P〈0.001),ODI(r=0.276,P〈0.001),and hs-CRP(r=0.182,P〈0.020). The multivariate Logistic regression analysis showed that RDW was one of the independent risk factors for OSA(OR=1.56,95%CI:1.231~2.012,P〈0.05). Conclusion The RDW could be used to indicate the diseaseseverity in patients with OSA.
