颅脑肿瘤术后并发远隔部位急性硬膜外血肿相关危险因素分析

Risk factors analysis of concurrent acute epidural hematoma at distant segment after craniocerebral tumor surgery

目的 探讨颅脑肿瘤术后并发远隔部位急性硬膜外血肿的相关危险因素.方法 将20例颅脑肿瘤术后并发远隔部位急性硬膜外血肿患者设为研究组,60例术后未出现硬膜外血肿患者设为对照组,对两组性别、年龄、脑积水程度、肿瘤大小、肿瘤位置、脑萎缩等临床资料进行对比分析,对研究组相关危险因素进行Logistic回归分析.结果 两组脑积水发生率、肿瘤大小、脑萎缩发生率比较差异均有极显著性（P〈0.05）.经Logistic回归分析显示,年龄、脑积水、肿瘤大小、脑萎缩为颅脑肿瘤术后并发远隔部位急性硬膜外血肿的危险因素（P〈0.01）.结论 年龄、脑积水、肿瘤大小、脑萎缩等是颅脑肿瘤患者术后并发远隔部位急性硬膜外血肿的危险因素,临床上应针对危险因素积极采取救治措施,提高治疗效果.

Objective To investigate the risk factors of concurrent acute epidural hematoma at distant segment after craniocerebral tumor surgery. Methods 20 patients who had concurrent acute epidural hemato- ma at distant segment after craniocerebral tumor surgery were selected as study group, 60 patients who did not have acute epidural hematoma were selected as control group. The clinical data, such as gender, age, degree of hydrocephalus, tumor size and location, encephalatrophy were compared between two groups. Logistic regression analysis of the risk factors in study group was performed. Results The incidence of hydrocephalus, tumor size and incidence of encephalatrophy were significantly different between two groups （P〈0.05）. The logistic regression analysis showed that age, hydrocephalus, tumor size, encephalatrophy were the risk factors for concurrent acute epidural hematoma at distant segment after craniocerebral tumor surgery （P〈0.01）. Conclusion The age, hydrocephalus, tumor size, encephalatrophy are the risk factors for concurrent acute epidural hematoma at distant segment after craniocerebral tumor surgery. Active measures have to be taken to control all the risk factors in order to improve the treatment effect.
