乳腺超声自动容积成像技术在乳腺癌早期诊断中的价值 被引量:17

The value of automatic breast volume scanner in the diagnosis of early breast cancer

摘要 目的:探讨最长径≤2cm的乳腺癌在乳腺超声自动容积成像(ABVS)中的图像特征,对比分析ABVS与普通超声(US)对≤2cm乳腺癌的诊断价值。方法:对114例患者共128个≤2cm的乳腺结节术前进行US及ABVS检查,所有病例均获得手术病理结果 ,对比分析二者图像特征及对≤2cm的乳腺癌的诊断价值。结果 :ABVS对乳腺癌边缘成角毛刺、肿块内簇点状微钙化显示阳性率分别为95.5%、54.5%,显著高于US显示阳性率69.7%、22.7%(均P<0.05)。US诊断乳腺癌敏感性、特异性和准确性为84.8%、69.4%、77.3%;ABVS为95.5%、72.6%、84.4%;US与ABVS二者结合为97.0%、75.8%和86.7%。ABVS诊断敏感性显著高于US(P<0.05),US与ABVS二者结合诊断敏感性显著高于US(P<0.05)。结论 :ABVS较普通超声能提供更丰富、清晰的图像,ABVS对早期乳腺癌诊断敏感性高于US,可作为有效手段用于乳腺癌早期诊断中。 Objective:To study the features of images of malignant breast lesions ≤2cm in automatic breast volume scanner(ABVS)system and analyze the diagnostic significances of ABVS compared with ultrasound(US) on breast leisions ≤2cm.Methods:ABVS and US examinations were performed to 114 patients with 128 breast leisions that were ≤2cm before surgery.We comparatively analyzed the characteristics of the images in ABVS system and in US.With the pathological diagnostic criteria,the diagnostic efficiencies were analyzed and contrasted.Results:In this study,the positive rates of the signs of speculated homed margin and clustered micro-calcifications in ABVS were 95.5% and 54.5% ,which were much higher than those in US 69.7% and 22.7%;the difference was statistically significant (P〈0.05).The sensitivity,specificity,accuracy positive of US were 84.8%,69.4% and 77.3%,which of ABVS were 95.5%,72.6% and 84.4%,which of combination of the two examinations were 97.0% ,75.8% and 86.7% ,respectively.The sensitivity rate of ABVS was significantly higher than that of US (P〈0.05).The sensitivity rate of combined examinations was significantly higher than that of US (P〈0.05).Conclusion:ABVS could provide more clear and richer images than US.The sensitivity rate of ABVS was higher than that of US.ABVS could be used as an effective examination mode in the diagnosis of early breast cancer.
