心脏局部肾素血管紧张素系统 被引量:2

摘要 肾素-血管紧张素系统(RAS)不仅在血压调节和体液、电解质平衡中起重要作用,还直接参与某些心脏病的病理生理变化,如心梗后心脏重塑和左室肥厚.以往一直把RAS作为一循环系统来看待,在盐、体液丢失或交感神经激活时,肾脏分泌肾素,继而作用于血中由肝产生的血管紧张素原(Aogen)使其转变成血管紧张素Ⅰ(AngⅠ),其后AngⅠ在血管紧张素转换酶(ACE)的作用下转变为血管紧张素Ⅱ(AngⅡ)--RAS的主要效应成分,AngⅡ则通过刺激其在靶细胞膜上的受体如AT1和AT2来发挥作用.自二十余年前开始应用RAS阻滞剂来控制血压,人们观察到血中RAS各成份浓度的变化与血压下降对心血管系统的影响不相符,遂提出所谓局部RAS的存在,其后研究表明大多数RAS成分可在组织生成,如脑、心、肾、血管等,组织RAS的概念已日趋成熟,即除了系统RAS外各种组织还有它们自己的RAS-AngⅡ可在组织中产生并在局部发挥作用.组织RAS的作用和其与系统RAS的关系尚在研究之中,组织RAS似乎通过旁分泌和/或自分泌方式来独立地影响器官功能.
