输尿管镜下气压弹道碎石术治疗输尿管中下段结石的临床效果分析

Analysis of clinical effect of ureteroscopic pneumatic lithotripsy in the treatment of middle and lower ureteral calculi

目的分析输尿管镜下气压弹道碎石术治疗输尿管中下段结石的临床效果。方法随机抽取2015年1月~2016年12月我院收治的50例输尿管中下段结石患者,按照抽签的方式将其分为病例组(n=25)和对照组(n=25)。对照组行体外冲击波碎石术(ESWL)治疗,病例组行输尿管镜下气压弹道碎石术(URSL)治疗。比较两组的治疗效果、不良反应发生率及生活质量状况(心理功能、物质生活、躯体功能及社会功能方面)。结果病例组的总有效率为96%,显著高于对照组的72%,差异有统计学意义(P<0.05)。病例组的不良反应发生率为8%,显著低于对照组的32%,差异有统计学意义(P<0.05)。病例组的心理功能、物质生活、躯体功能及社会功能评分显著高于对照组,差异有统计学意义(P<0.05)。结论 URSL治疗输尿管中下段结石的临床效果显著,患者的病情明显好转,能够一次性清除输尿管结石,解除梗阻,改善肾功能,不良反应发生率极低,生活质量明显提高,更具安全性和可行性等优点,值得借鉴和推广。

Objective To analyze the clinical effect of ureteroscopic pneumatic lithotripsy（URSL） in the treatment of middle and lower ureteral calculi.Methods Altogether 50 patients with middle and lower ureteral calculi who were treated in our hospital from January 2015 to December 2016 were selected and divided into the case group（n=25） and the control group（n=25） by drawing lots.The control group was given extracorporeal shock-wave lithotripsy（ESWL） treatment,while,the case group were treated with URSL.The treatment effect,the incidence rate of adverse reaction and quality of life（psychological functions,material life,physical functions and social functions） of the two groups were compared.Results The total effective rate of the case group was 96%,which was significantly higher than 72% of the control group,with significant difference（P〈0.05）.The incidence rate of adverse reactions in the case group was 8%,which was significantly lower than 32% in the control group,with significant difference（P〈0.05）.The scores of psychological function,material life,physical function and social function in the case group was significantly higher than that in the control group,with significant difference（P〈0.05）.Conclusion The clinical effect of URSL in the treatment of middle and lower ureteral calculi is remarkable,with significantly improved patients′ conditions and one-time removal of ureteral calculi for obstruction relief,which improves renal functions and quality of life and has low incidence of adverse reactions and multiple advantages such as more safety and feasibility,which is worth learning and promotion.
