椎弓根内固定结合经椎弓根植骨治疗脊柱爆裂骨折的疗效

Curative Effect of Trans-pedicle Internal Fixation and Transpedicular Bone Grafting in Treatment of Spinal Burst Fracture

目的探讨椎弓根内固定结合经椎弓根植骨治疗脊柱爆裂骨折的疗效。方法方便收集2012年4月—2016年12月该院胸腰段脊柱爆裂骨折的100例患者,Frankel分级C、D、E级。随机分为:研究组(接受椎弓根内固定结合经椎弓根植骨治疗)和对照组(接受椎弓根内固定治疗)。对比两组术后6个月VAS评分及Frankel分级。以及两组术后6个月椎体压缩率及Cobb角。结果研究组和对照组术后6个月Frankel分级(B级、C级、D级、E级)例数分别为(0例、0例、2例、48例)、(0例、0例、9例、41例),比较差异有统计学意义(P<0.05)。研究组和对照组术后6个月椎体压缩率、Cobb角及VAS评分分别为[(10.36±2.74)%、(4.36±1.87)°、(2.64±0.23)分]、[(14.37±2.85)%、(6.15±2.14)°、(3.61±0.34)分]比较差异有统计学意义(P<0.05)。结论椎弓根内固定结合经椎弓根植骨治疗脊柱爆裂骨折的疗效肯定,术后椎体压缩率及Cobb角丢失率少,VAS评分及Frankel分级优。

Objective To discuss the curative effect of trans-pedicle internal fixation and transpedicular bone grafting in treatment of spinal burst fracture. Methods Convenient selection 100 cases of patients with thoraclumbar burst fracture in our hospital from April 2012 to December 2016 were selected and randomly divided into the two groups, and the Frankel classification was C,D,E, and the research group adopted the trans-pedicle internal fixation and transpedicular bone grafting, while the control group adopted the trans-pedicle internal fixation, and the VAS score at different time and Frankel classification were compared between the two groups and the vertebral compression rates and Cobb angles at different time after surgery were compared between the two groups. Results The differences in the cases in B level, C level, D level and E level of Frankel between the research group and the control group were statistically significant（0 cases, 0 cases, 2 cases, 48 cases vs 0 cases, 0 cases, 9 cases, 41 cases）（P〈0.05）, and the differences in the vertebral compression rate in 6 months after surgery, Cobb angle and VAS score between the research group and the control group were statistically significant[（10.36±2.74）%,（4.36±1.87）°,（2.64±0.23）points vs（14.37±2.85）%,（6.15±2.14）°,（3.61±0.34）points]（P〈0.05）. Conclusion The curative effect of trans-pedicle internal fixation and transpedicular bone grafting in treatment of Spinal burst fracture is well received and the postoperative vertebral compression rate and Cobb angle loss rate are fewer and the VAS score and Frankel level are better.
