实时三维经食管超声在房间隔缺损介入封堵术前的价值

Value of RT-3D TEE in Transcatheter Closure of Atrial Septal Defect

目的:探讨实时三维经食管超声心动图(RT-3D TEE)在房间隔缺损(ASD)介入封堵术前的价值。方法:35例ASD患者分别应用二维经食管超声(2D-TEE)、RT-3D TEE观察ASD形态特征、与周围结构位置的关系、测量ASD最大径、将适合经导管房间隔缺损封堵的患者两种方法的测量结果相比较并分析其与封堵器大小的相关性。结果:RT-3D TEE能够直观判断ASD的类型、数目、边缘情况,明确指导治疗方法,在心室收缩末期可测得最大径,与封堵器大小的相关性高于2D-TEE。结论:RT-3D TEE在ASD的诊断中能够在形状、结构和毗邻关系等判断方面提供ASD的形态特征的三维立体视角,适合介入封堵术的患者的治疗。

Objective;To evaluate the value of real-time three-dimensional transesophageal echocardiography（RT-3D TEE） in transcatheter closure of atrial septal defect（ASD）.Methods;35 cases of ASD patients were using two-dimensional transesophageal echocardiography（2D-TEE）,RT-3D TEE ASD to observe the morphological characteristics and relationship with the surrounding structures,position measurement of ASD size,the measurement results will be suitable for transcatheter closure of atrial septal defects in patients with two kinds of methods are compared and analyzed the correlation with the size of the occluder.Results：RT-3D TEE can directly determine the type,number,edge ASD,clear guidance in treatment of ventricular end systolic diameter can be measured,and the correlation between the size of occluder was higher than that of 2D-TEE.Conclusion：RT-3 D TEE can be used in the diagnosis of ASD in the shape,structure and the relationship between the adjacent areas to provide a three-dimensional perspective of the morphological characteristics of ASD,suitable for interventional occlusion of patients with treatment.
