呼吸机相关性肺炎并发假丝酵母菌感染患者PCT、IL-6和CRP水平分析 被引量:15

Analysis of PCT,IL-6 and CRP levels in patients with ventilator-associated pneumonia and Candida infections

摘要 目的探究呼吸机相关性肺炎(VAP)并发单一假丝酵母菌感染患者的临床特征以及与降钙素原(PCT)、白细胞介素-6(IL-6)和C-反应蛋白(CRP)等炎症指标的相关性。方法选择2008年1月-2016年3月医院收治的149例VAP患者,根据假丝酵母菌感染的相关诊断标准,分为假丝酵母菌属组65例、其他病菌组84例;比较两组患者临床指标。结果在65例伴有假丝酵母菌感染的VAP患者中,以白假丝酵母菌感染为主,共40株占61.54%;在假丝酵母菌属组中,β-葡聚糖阳性患者的PCT、IL-6以及CRP水平均显著高于β-葡聚糖阴性患者,组间比较差异有统计学意义(P<0.05)。结论导致VAP患者发生的假丝酵母菌感染的易感因素众多,在临床治疗期间需积极消除各种易感因素,及时给予其相应的抗真菌感染治疗;β-葡聚糖在评价假丝酵母菌感染以及感染程度上具有一定的临床应用价值。 OBJECTIVE To investigate the clinical characteristics of patients with ventilator-associated pneumonia(VAP)and Candidainfections and its correlations with PCT,IL-6and CRP.METHODS A total of 149 cases of VAP patients from Jan.2008 to Mar.2016 in the hospital were selected,and were divided into 65 cases of Candida group and 84 cases of other bacteria group according to the diagnostic criteria of Candidainfections.The clinical indexes of the two groups of patients were compared.RESULTSIn 65 cases of VAP patients with Candidainfections,the most were Candida albicans infection,which were 40 strains,accounting for 61.54%.In Candida group,of the levels of PCT,IL-6 and CRP of beta-glucan positive group were significantly higher than those of beta-glucan negative group(P〈0.05).CONCLUSIONThere are many predisposing factors of Candida infection in patients with VAP,which should be actively eliminated during clinical treatment,and the appropriate antifungal therapy should be given in time.Beta-glucan has some clinical application values in the evaluation of Candidainfections and infection degree.
