摘要
目的观察幽门螺旋杆菌(Hp)感染脑梗死患者血液流变学和血流动力学的特点,分析Hp感染对于脑梗死的影响。方法选取医院2014年1月-2016年2月间确诊的93例脑梗死伴有Hp感染病例作为观察组,且根据Hp感染类型分为Ⅰ型亚组患者54例和Ⅱ型亚组患者39例,同时选取60例未感染Hp的脑梗死病例作为病例对照组,选取同期体检55例健康成年人作为健康对照组,分析各组间在血液流变学、血流动力学方面的异同点。结果观察组血浆黏度、全血黏度、全血还原黏度、红细胞聚集指数、红细胞压积显著高于健康对照组和病例对照组(P<0.05),且病例对照组以上指标显著高于健康对照组(P<0.05);Ⅰ型亚组以上指标均显著高于Ⅱ型亚组,且大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)、椎动脉(VA)、颈内动脉末端(TICA)血流速度以及红细胞变形指数显著低于Ⅱ型亚组(P<0.05)。结论 Hp感染能够影响脑梗死患者的血流变及血流动力,其可能与脑梗死发病机制具有密切相关性。
OBJECTIVE To observe the characteristics of hemorheology and hemodynamics of patients with cerebral infarction and Helicobacter pylori(Hp)infection and analyze the effects of Hp infection on cerebral infarction.METHODS A total of 93 patients with cerebral infarction and Hp infection in our hospital from Jan.2014 to Feb.2016 were retrospectively chosen as observation group.According to the type of Hp infection,they were divided into 54 cases of typeⅠ subgroup and 39 cases of typeⅡ subgroup.At the same time,60 cases of patients with cerebral infarction and without Hp infection were selected as cases control group,and 55 healthy adults were selected as health control group.The differences and similarities of hemorrheology and hemodynamics between the groups were analyzed.RESULTS The plasma viscosity,whole blood viscosity,whole blood reduction viscosity,erythrocyte aggregation index,and hematocrit in observation group were significantly higher than those in healthy control group and case control group(P〈0.05),and the above indicators of case control group were significantly higher than those of healthy control group(P〈0.05).The above indicators of typeⅠsubgroup were significantly higher than those of typeⅡ subgroup,and the middle cerebral artery,anterior cerebral artery,posterior cerebral artery,vertebral artery,the end of carotid artery,blood flow velocity and erythrocyte deformation index of typeⅠsubgroup were significantly lower than those of typeⅡsubgroup(P〈0.05).CONCLUSION Hp infection can affect the hemorheology and hemodynamics of patients with cerebral infarction,which may be closely related to the pathogenesis of cerebral infarction.
