术后盆腔感染诊断中超声的应用分析

Application of ultrasonography in the diagnosis of postoperative pelvic infections

目的探讨超声在术后盆腔感染诊断中的应用价值,为术后盆腔感染的诊断提供参考。方法选取2016年1-10月期间临床拟诊为术后盆腔感染的100例患者,行超声检查,并与确诊结果进行比较。结果确诊盆腔感染92例占92.00%;超声检查71例诊断盆腔感染占71.00%,其中30例输卵管炎占30.00%、15例子宫内膜炎占15.00%、4例宫腔积脓占4.00%、6例输卵管炎合并子宫内膜炎占6.00%、16例盆腔脓肿占16.00%、8例卵巢囊肿占8.00%、2例子宫肌瘤占2.00%、3例恶性肿瘤占3.00%、12例单纯盆腔积液占12.00%、4例未见异常占4.00%;超声诊断术后盆腔感染的敏感度为73.91%、特异度为62.50%、阳性预测值为95.77%、诊断符合率为73.00%。结论超声用于术后盆腔感染的诊断敏感度及特异性较低,超声下表现无特异性,容易误诊为其他疾病,应结合病理及其他检查以提高诊断的准确性。

OBJECTIVE To investigate the value of ultrasonography in the diagnosis of postoperative pelvic infections,so as to provide reference basis for the diagnosis of postoperative pelvic infections.METHODS A total of100 patients who were diagnosed as postoperative pelvic infections form Jan.to Oct.2016 were selected.Patients were carried out ultrasonography,and were compared with the results of diagnosis.RESULTS Totally 92 cases of pelvic infections were diagnosed,accounting for 92.00%.Totally 71 cases of pelvic infection were diagnosed by ultrasonography,accounting for 71.00%,including 30 cases of salpingitis（30.00%）,15 cases of endometritis（15.00%）,4cases of uterine empyema（4.00%）,6cases of salpingitis combined with endometritis（6.00%）,16 cases of pelvic abscess（16.00%）,8cases of ovarian cyst（8.00%）,2cases of uterine fibroid（2.00%）,3cases of malignant tumor（3.00%）,12 cases of pure pelvic effusion（12.00%）,and 4cases of no abnormity（4.00%）.In the diagnosis of postoperative pelvic infection by ultrasonography,the sensitivity was 73.91%,specificity was62.50%,positive predictive value was 95.77%,and the diagnostic coincidence rate was 73.66%.CONCLUSION Diagnostic sensitivity and specificity of ultrasonography used for postoperative pelvic infection are low,the ultrasonographic findings have no specificity and can be easily misdiagnosed as other diseases,which should be combined with diseases and combined with other examinations in order to improve the accuracy of diagnosis.
