腹腔镜与开腹手术治疗结肠癌疗效对比研究

Research on Comparison of Curative Effect of Laparoscope and Open Surgery in Treatment of Carcinoma of Colon

目的探讨腹腔镜与开腹手术在结肠癌治疗中的效果。方法对该科2013年4月—2016年4月收治的144例结肠癌进行手术治疗,随机分为两组,腹腔镜组采用腹腔镜手术治疗,开腹组采用传统开腹手术治疗,每组各72例,观察两组术中、术后及并发症发生、随访情况。结果腹腔镜组手术时间(159.4±36.7)min、出血量(137.5±12.1)m L、排气时间(15.7±2.1)h、下床时间(2.4±1.1)d、住院时间(5.4±1.3)d均明显优于开腹组手术时间(202.9±31.4)min、出血量(217.6±26.8)m L、排气时间(25.7±1.3)h、下床时间(5.8±1.4)d、住院时间(11.6±2.7)d(P<0.05)。腹腔镜组并发症发生率为5.6%,开腹组并发症发生率为19.4%,两组比较差异有统计学意义(P<0.05);患者术后随访平均(12.5±3.7)个月,腹腔镜组死亡率为2.8%,开腹组死亡率为4.2%,两组比较差异无统计学意义(P>0.05)。结论腹腔镜作为治疗结肠癌的主要术式具有创伤小、恢复快、并发症少等优点,有效促进了患者的恢复,提高了患者的生活和生命质量,值得应用。

Objective To discuss the effect of laparoscope and open surgery in treatment of carcinoma of colon. Methods 144 cases of patients with carcinoma of colon admitted and treated in our department in our hospital from April 2013 to April 2016 were selected and randomly divided into two groups with 72 cases in each, the laparoscope group were treated with laparoscope, while the open surgery group adopted the open surgery, and the 6ccurrence of intraoperative and postoperative complications and followup of the two groups were observed. Results The operation time, bleeding amount, exhaust time, time to get out of bed and length of stay in the laparoscope group were obviously better than those in the open surgery group[（159.4±36.7）min, （137.5±12.1）mL, （15.7±2.1）h, （2.4±1.1）d, （5.4±1.3）d vs （202.9±31.4）min, （217.6±26.8）mL, （25.7±1.3）h, （5.8±1.4）d, （11.6±2.7）d], and the difference in the incidence rate of complications between the laparoscope group and the open surgery group had statistical significance（5.6% vs 19.4%）, and the postoperative followup time was （12.5±3.7）months, and the difference in the death rate between the laparoscope group and the open surgery group had no statistical significance （2.8% vs 4,2%）（P〉0.05）. Conclusion Laparoscope, as a main operation method, has the advantages of less wound, rapid recovery and few complications, which can effectivelypromote the recovery of patients and improve the quality of life of patients, and it is worth application.
