超声检查在不同月龄婴幼儿发育性髋关节脱位诊断中的应用研究 被引量:15

摘要 目的:探讨超声在婴幼儿发育性髋关节脱位(Develepment Dislocationg of the Hip,DDH)中的应用,并研究超声检查适合的月龄组。方法:将就诊的高危DDH患儿人群按月龄分为4组,(80例,160髋)。A组为0~3mo;B组为4~6mo;C组为7~9mo;D组为10~12mo。分别进行双侧髋关节超声检查(Graf法)及骨盆正位X线平片检查。并对其阳性检出率进行比较。结果:A组:超声检查阳性率高于普通X线检查,P<0.05,差异有统计学意义。B组:超声检查阳性率与普通X线检查阳性率无差别,P>0.05,差异无统计学意义。C、D组:普通X线检查阳性率高于超声检查,P<0.05,差异有统计学意义。结论:在DDH的诊断上,超声波能够更加早期地发现髋关节脱位的征象。超声波检查方法可以显示尚未骨化的新生儿和小婴儿时期的髋关节结构。适用于6mo以下的儿童,特别适用于3mo以下的婴幼儿,超声波没有电离辐射危害,具有独特的影像学的优势。 Objective:To investigate the application of ultrasound in infants and young children with developmental dislocation of the hip( Develepment Dislocationg of the Hip,DDH) and to study ul-trasound suitable month group. Methods:A high-risk populations in DDH divided into four groups by months(80cases,160hips). Group A:0-3months;group B:4-6months;group C:7-9months;group D:10-12months. They were bilateral hip ultrasonography(Graf method)and pelvic anteroposterior X-ray examination. And compare its positive rate. Results:Group A:Ultrasonography was higher than ordinary X-ray examination,P&lt;0. 05,the difference was statistically significant. Group B:The positive rate of ultrasound and conventional X-ray examination positive rate of no difference, P〉0. 05, the difference was not statistically significant. Group C、D:General X-ray examination was higher than ul-trasound,P&lt;0. 05,the difference was statistically significant. Conclusion:In the diagnosis of DDH, ultrasound can be found earlier signs of dislocation of the hip. Ultrasonic inspection method is an effective means yet ossification hip structure in neonates and young infants during the show. For less than 6months of age,especially in children under 3months of age,ultrasound can clearly demonstrate the advantages of imaging,and can reduce infant ionizing radiation hazards.
