硬膜外型颈椎间盘突出症的临床分析及相关解剖学研究

目的针对硬膜外型颈椎间盘突出症进行临床分析,并在此基础上进行相关解剖学研究。方法选取该院2015年3月—2016年1月期间接收并治疗的150例颈椎间盘突出患者作为该次研究对象,针对患者的头颈部外伤史,以及上下肢的症状等,利用X线以及MRI进行检查,患者均行颈椎前路手术实施治疗,同时结合患者的病症情况等选择单纯的颈间盘解切除术或者椎体切除术等。对比患者治疗前后的功能恢复状况。结果该次研究中150例患者接受手术治疗前,神经功能优良率为0.00%,接受手术治疗后神经功能优良率提升到70.00%,其中神经功能为优42例(28.00%),神经功能为良63例(42.00%),神经功能为可34例(22.67%),神经功能差11例(7.33%),神经功能优良率显著提升,患者手术治疗前后差异有统计学意义(P<0.05)。结论硬膜外型颈椎间盘突出症是颈椎间盘突出症中的特殊类型,发病原因与患者头颈部受到外伤损害有关,通过手术治疗效果确切,手术治疗方式是改病症的优选治疗方式。

Objective To carry out the clinical analysis of epidural cervical eisc herniation and related anatomy research. Methods 150 cases of patients with epidural cervical disc herniation admitted and treated in our hospital from March 2015 to January 2016 were selected as the research objects and the head and neck trauma history and upper and lower limb symptoms were examined by the X ray and MRI, and all patients were treated with anterior cervical surgery, and the simple cervical intervertebral disc resection or corpectomy of patients was selected combined with the disease of patients, and the function recovery of patients before and after treatment was compared. Results In the 150 cases of patients, before the operative treatment, the excellent and good rate of nerve function increased from 0.00% to 70.00% after treatment, and there were 42 cases whose nerve function was excellent（28.00%）, 63 cases whose nerve function was good （42.00%）, 34 cases whose nerve function was fair（22.67%） and 11 cases whose nerve function was bad, （7.33%）, and the excellent and good rate of the nerve function obviously increased, and the difference before and after treatment had statistical significance（P〈0.05）. Conclusion The epidural cervical disc herniation is a special type of herniation of cervical disc, and the pathogenesis is related to the head and neck trauma injury, and the operative treatment effect is definite, and the operative treatment method is a preferred method.
