初产妇采用导乐球配合自由体位促进自然分娩的效果

目的探讨导乐球配合自由体位在促进初产妇自然分娩中的作用。方法收集2015年2月至2016年8月初产妇106例，随机分为观察组与对照组，各53例。观察组予以导乐球配合自由体位助产，对照组予以传统卧位分娩。观察两组产后出血情况，比较自然分娩率与产程时间等指标。结果观察组自然分娩率96．23％，对照组88．68％。观察组明显高于对照组，差异有统计学意义（P〈0．05）。观察组总产程时间（429．73±97．46）min明显比对照组（508．06±129．74）min缩短．产后出血率1．89％明显比对照组9．43％降低，差异有统计学意义（P〈0．05）。新生儿娩出后Imin与5min时Apga晰分，观察组明显高于对照组，差异有统计学意义（P〈005）。结论采取导乐球配合自由体位法，对初产妇进行助产，可显著提高自然分娩率，降低产后出血发生风险，确保新生儿健康。

Objective To analyze the effect of Doula ball combined with free position in the beginning of promoting natural delivery. Methods From February 2015 to August 2016, a total of 106 cases of primipara admitted to our department were randomly divided into test and control group, 53 patients in each group. The test group received the Doula ball midwifery with the free position of Doula midwifery, the control group received traditional supine delivery. Two groups of postpartum hemorrhage were observed and compared with the natural delivery rate and labor time. Results The natural delivery rate of the experimental group was 96.23%, and the control group was 88.68%. The experimental group was significantly higher than that of the control group. The difference was statistically significant between the two groups （ P〈0.05 ） . The total labor time （ 429.73 ± 97.46 ） rain in the experimental group was significantly shorter than that in the control group （ 508.06± 129.74 ） min, the rate of postpartum hemorrhage was 1.89% lower than 9.43% of the control group. The difference was statistically significant between the two groups （ P〈0.05 ） . From neonatal lmin and 5min of the Apgar score, the test group was significantly higher than the control group. The difference was statistically significant between the two groups （ P〈0.05 ） . Conclusion Doula ball combined with free position delivery primipara can significantly improve the rate of natural childbirth, reduce postpartum hemorrhage risk, ensure neonatal health.
