重度子痫前期孕妇早孕期及晚孕期甲状腺功能的研究分析 被引量:2

摘要 目的探究重度子痫前期孕妇早孕期及晚孕期甲状腺功能的变化以及与重度子痫前期发病的关联。方法选取2015年3月—2016年3月于该院就诊的重度子痫前期孕妇280例,并随机抽取同一时段在该院就诊的无产科综合征的健康孕妇280例作为对照组,采取化学发光微粒免疫分析技术测定两组孕妇血清中的TSH、FT3以FT4的水平,采取化学发光法测定TGAb和TPOAb的水平,比较分析两组孕妇的甲状腺功能。结果重度子痫前期孕妇晚孕期的TSH水平为(4.12±1.25)m U/L,明显高于健康孕妇对照组(1.72±0.81)m U/L;重度子痫前期孕妇晚孕期的FT3水平(2.82±0.70)pmol/L、FT4水平(12.40±2.26)pmol/L均明显低于健康孕妇对照组水平,且与对照组及早孕期相比,差异有统计学意义(P<0.05)。重度子痫前期孕妇晚孕期的TGAb阳性率(10.71%)、TPOAb阳性率(10.36%)和甲亢/亚临床甲亢的发生率(40.71%)均明显高于健康孕妇对照组,且与对照组及早孕期相比,差异均有统计学意义(P<0.05)。结论重度子痫前期孕妇常伴有甲状腺功能的变化,其中以甲减及亚临床甲减较为多见,孕妇重度子痫前期疾病的发生与其甲状腺功能的异常有关联。 Objective To investigate the changes of thyroid function in pregnant women diagnosed with early and late onset severe preeclampsia and to analyze their relation. Methods A total of 280 patients with severe pre-eclampsla of pregnant women were selected from March 2015 to March 2016 in out,hospital, while 280 healthy women with no ob- stetrics complications were selected as controls. The levels of FT3, FT4 and TSH were measured by chemiluminesent microparticle immunoassay respectively and the levels of TGAb and TPOAb were determined by method, the thyroid function was observed and compared between the two groups in pregnant women. Results Levels of TSH in severe pre-eclampsia of pregnant women (4.12±1.25)mU/L were higher than that in controls, and levels of FT3 (2.82±0.70)pmol/L, FT4(12.40±2.26)pmol/L in severe pre-eclampsia of pregnant women were lower than that in con- trois, the differences were statistically significant (P〈0.05); TGAb positive rate (10,71%), TPOAb positive rate (10.36%) and the incidence rate of hypothyroidism or subclinical hypothyroidism (40.71%) in severe pre-eclampsia of pregnantwomen were higher than that in controls, the differences were statistically significant (P〈0.05). Conclusion Pregnant women with severe pre-eclampsia women were often accompanied by the change of thyroid function, the incidence rate of hypothyroidism or subclinical hypothyroidism were the most. Incidence of severe preeclampsia disease may be related to abnormal thyroid function.
