摘要
目的:探讨CD4^+CD25^+Foxp3^+Tregs在自身免疫性甲状腺疾病(AITD)中的作用。方法:收集2014年2月-2016年11月在本院进行检查的AITD患者80例为治疗组,其中Graves(GD)患者40例,为GD组;桥本甲状腺炎(HT)患者40例,为HT组;选取健康体检人员20例为对照组。采用流式细胞术检测外周血CD4^+T、CD4^+CD25^+Tregs细胞数量和比例,采用RT-PCR检测外周血Foxp3 mRNA表达水平,采用化学发光法对血清中甲状腺素(TSH)、总三碘甲状腺原氨酸(TT_3)、总甲状腺素(TT_4)、甲状腺球蛋白抗体(TgAb)和甲状腺过氧化酶抗体(TPOAb)的水平进行测定。结果:三组TSH、TT_3、TT_4、TgAb、TPOAb比较,差异均有统计学意义(P<0.05);三组外周血CD4^+T细胞数量比较,差异无统计学意义(P>0.05);GD组外周血CD4^+CD25^+Tregs占CD4^+T细胞的比率与对照组比较,差异无统计学意义(P>0.05);HT组外周血CD4^+CD25^+Tregs占CD4^+T细胞的比率明显低于对照组,差异有统计学意义(P<0.05);GD组和HT组外周血Foxp3 mRNA表达量均明显低于对照组,差异均有统计学意义(P<0.05);自身免疫性甲状腺疾病的CD4^+CD25^+Foxp3^+Tregs与TSH、TT_3、TT_4、TgAb和TPOAb有明显正相关性(P<0.05)。结论:CD4^+CD25^+Foxp3^+Tregs可能在AITD的发生发展进程中起着一定的作用,深入研究CD4^+CD25^+Foxp3^+Tregs在AITD中的作用将有助于建立新的预防和治疗策略。
Objective: To study the role of CD4^+CD25^+Foxp3^+Tregs in autoimmune thyroid diseases ( AITD ) .Method: 80 cases of AITD were collected from February 2014 to November 2016 in our hospital.Included 40 patients with Graves ( GD ), as GD group, 40 patients with Hashimoto's thyroiditis ( HT ), as HT group, 20 cases healthy people were selected as the control group.The peripheral blood flow Cytometric detection of CD4^+T and the number and proportion of CD4^+CD25^+Tregs calls were detected by flow cytometry, the peripheral blood Foxp3 expression level was detected by RT-PCR, the levels of total thyroxine ( TSH ), total three triiodothyronine ( TT3 ), total thyroxine ( TT4 ), thyroglobulin antibody ( TgAb ) and thyroid peroxidase ( TPOAb ) were measured by chemiluminescence method.Result: TSH, TT3, TT4, TgAb, TPOAb levels of three groups had significant differences ( P〈0.05 ) .The peripheral blood of CD4^+T of three groups had no significant difference (P〉0.05 ) . Peripheral blood CD4^+CD25^+Tregs cells and the ratio of CD4^+T of GD group compared with the control group, there were no significant differences (P〉0.05 ) .Peripheral blood CD4^+CD25^+Tregs cells and the ratio of CD4^+T of HT group were significantly lower than those of the control group ( P〈0.05 ) .Foxp3 mRNA expression of GD group and HT group were significantly lower than that the control group ( P〈0.05 ) .CD4^+CD25^+Foxp3^+Tregs of AITD was positively correlated with TSH, TT3, TT4, TgAb and TPOAb (P〈0.05) .Conclusion: CD4^+CD25^+Foxp3^+Tregs may play an important role in the occurrence and development of AITD.Further research on the role of CD4^+CD25^+Foxp3^+Tregs in AITD will help to establish a new prevention and treatment strategy.
