组织多普勒超声评价收缩后收缩对急性心肌梗死年轻患者的诊疗价值

The clinical value of postsystolic shortening in the diagnosis of acute myocardial infarction of youth patients by tissue Doppler ultrasonography

目的利用组织多普勒超声测定收缩后收缩(PSS)评价急性心肌梗死(AMI)年轻患者,探讨其临床应用价值。方法选取年轻AMI患者44例设为AMI组,44名年轻正常人设为对照组;根据AMI患者是否出现PSS,将AMI组分为非PSS组(NO-PSS组)(n=25)和PSS组(n=19)。利用组织多普勒超声检测AMI组和对照组的局部心肌。结果与对照组相比,AMI组患者心腔大小以及心功能改变不明显(P>0.05);PSS组冠状动脉病变血管的狭窄程度和SYNTAX积分显著高于NO-PSS组(P<0.05);PSS组患者E、A、E/A、E峰减速度、等容舒张时间、Ve和Va与NO-PSS组比较,差异均存在统计学意义(P<0.05)。结论组织多普勒超声评价PSS对AMI年轻患者诊断的临床应用效果较好,值得推广和应用。

Objective To evaluate the clinical value of the postoperative contraction （PSS） by tissue Doppler uhrasonography in the diagnosis of acute myocardial infarction （AMI） in young patients. Methods Fourty-four cases of young normal subjects were selected as control group, and 44 young AMI patients were selected as AMI group. The AMI patients were divided into group NO-PSS （n=25） and group PSS （n=19） according to whether the patient appeared PSS or not. The local myocardium was detected by tissue Doppler ultrasonography in the AMI group and control group. Results There was no significant difference in the cardiac cavity size and cardiac function between AMI group the control group （P〉0.05）. The degree of stenosis in the coronary arteries and SYNTAX scores in group PSS were significantly higher than those in group NO-PSS （P〈0.05）. There were significant differences in E, A, E/A, E peak deceleration, isometric relaxation time, Ve and Va between group NO-PSS and group PSS （P〈0.05）. Conclusion Tissue Doppler ultrasonography has a good clinical effect in the diagnosis of PSS in young patients with AMI. It is worthy to be popularized and applied.
