氯磷定救治急性有机磷农药中毒用药方法研究

Medication methods research of pralidoxime chloride in the treatment of acute organophosphate poisoning

目的研究更有效使用氯磷定救治急性有机磷农药中毒(AOPP)的用药方法。方法回顾性分析96例AOPP患者的临床资料,根据氯磷定用药方法的不同分为治疗组(n=56)和对照组(n=40)。治疗组患者给予氯磷定电脑输液泵持续泵入,对照组患者应用氯磷定肌肉注射。比较两组患者的临床效果。结果中、重度中毒时,治疗组患者胆碱酯酶活力恢复正常时间及氯磷定用量明显优于对照组,差异具有统计学意义(P<0.05)。治疗组患者严重并发症发生率、住院时间、阿托品用量明显优于对照组,差异具有统计学意义(P<0.05)。结论氯磷定电脑输液泵持续泵入救治AOPP,操作方便,可控性强,顺从性好,能提高疗效,缩短住院时间,是更有效的用药方法,值得临床推广应用。

Objective To study the medication method of pralidoxime chloride in the treatment of acute organophosphate poisoning （AOPP）. Methods The clinical data of 96 cases of AOPP patients were retrospectively analyzed, all the patients were divided into treatment group （n=56） and control group （n=40） according to different medication methods. The treatment group received continuous infusion of pralidoxime chloride through computerized infusion pump, and the control group was treated with pralidoxime chloride through intramuscular injection. The clinical effects of the two groups were compared. Results For moderate and severe poisoning, the cholinesterase activity recovery time and the pralidoxime chloride dose of the treatment group were better than those of the control group （P〈0.05）. Compared with the control group, the incidence of severe complications, hospital stays and atropine dose in the treatment group were better, the differences were statistically significant （P〈0.05）. Conclusion Continuous injection of pralidoxime chloride by computerized infusion pump has the advantages of convenient operation, strong controllability and good compliance, and that can improve the curative effect and reduce the hospitalization day. It is a more effective medication method, which is worth application and popularization in clinic.
