2AM眼压在24小时眼压测量中的意义

摘要 目的评价2AM眼压测量在24小时眼压测量中意义。方法回顾性分析2014年4月至7月天津市眼科医院30例24小时眼压测量患者结果。采用Goldmann压平眼压计,按青光眼学组最新提出的10AM,2PM,6PM,10PM,2AM,5AM,7AM的7次眼压测量法坐位测量。收测患者30例60只眼,男10例,女20例;年龄18-69岁,平均36.3±14.8岁;入院诊断可疑青光眼未用药者20例,入院已经确定诊断青光眼,观察用药眼压者10例。结果2AM眼压为测量眼峰值眼压者共9/60只眼(15%),为谷值眼压者18/60只眼(30%)。2AM与另6次眼压均值配对t检验差异无统计学意义(t=-1.879,P=0.065);两者有较好相关性,r=0.810。结论2AM眼压可以反映15%的峰值眼压,30%的谷值眼压,具有临床意义,应该给予坚持。 Objective To judge the clinical value of 2AM IOP testing in 24 hour IOP monitor- ing. Methods A retrospectively study of 30 cases(60 eyes) of the 24 hour IOP monitoring records in Tianjin Eye hospital from April to July,2014, including 10 males and 20 females, age from 18 to 69, average 36.3±14.8. Goldmann applanation tonometer, sitting position, 7 times lOP measurement at 10AM,2PM,6PM,10PM,2AM,5AM,TAM, were used for statistical analysis. Results 9/60 eyes (15%)of 2AM IOP were peak IOP of the 24-hour fluctuation and 18/60 eyes (30% ) were trough. There was no significant difference between 2AM IOP and the average IOP of the other 6 times (t =-1.879,P =0.065). High positive correlation were found between 2AM IOP and the average IOP of the other 6 times, r =0.810. Conclusions 2AM lOP has about 15% peak IOP and 30% trough IOP of the 24 hours IOP, and therefore, 2AM IOP should be insist on.
