内镜下切开术联合探条扩张术与单纯扩张法在食管良性狭窄治疗效果比较

The effect of endoscopic incision combined with bouginage and simple dilatation in the treatment of benign esophageal stricture

[目的]分析内镜下切开术联合探条扩张术与单纯扩张法在食管良性狭窄的临床应用疗效。[方法]选取2014年7月~2016年7月我院收治150例食管良性狭窄患者作为研究对象,按照随机数字表将其分为对照组(n=75)及实验组(n=75),对照组采用单纯扩张法进行治疗,实验组采用内镜下切开术联合探条扩张术进行治疗,对比2组临床治疗效果、持续症状缓解时间、再次进行内镜下扩张治疗的间隔时间和并发症等情况。[结果]实验组的总有效率明显高于对照组(96.00%:84.00%,P<0.05);实验组的持续症状缓解时间明显长于对照组[(173.35±9.13)d:(101.33±7.82)d],P<0.05;实验组的再次进行内镜下扩张治疗的间隔时间明显长于对照组(189.23±20.15)d:(107.92±15.21)d,P<0.05),2组均无严重并发症发生。[结论]在食管良性狭窄治疗方面,采用内镜下切开术联合探条扩张术治疗方案,临床效果显著,能够有效改善食管狭窄情况,大大提高患者的生活质量,值得推广。

[Objective]To analyse the clinical effect of endoscopic incision combined with bouginage and simple dilatation in the treatment of benign esophageal stricture.[Methods]150cases of patients with benign esophageal stricture from July 2014 to July 2016 were randomly divided into control group（n=75）and experimental group（n=75）.The control group was treated by simple dilatation while the experimental group was treated by endoscopic incision combined with bouginage.The clinical effect,the sustained remission time,the time interval for retreatment by dilatation and complication were compared between the two groups.[Results]The effective percentage of the experimental group was higher than the control group（96.00% and 84.00%,P〈0.05）;The sustained remission time of the experimental group was longer than the control group（173.35d±9.13 d and 101.33d±7.82 d,P〈0.05）;The time interval for retreatment by dilatation of the experimental group was longer than the control group（189.23 d±20.15 d and 107.92d±15.21 d,P〈0.05）;There was no severe complication in the two groups.[Conclusion]The treatment of benign esophageal stricture by endoscopic incision combined with bouginage has a significant clinical effect,improving the esophageal stricture effectively,greatly enhancing the patients＇ quality of life,which was worth to spread.
